摘要
目的:探讨血清甲状腺激素及外周血白细胞计数在急性心肌梗死心力衰竭严重程度评估中的作用。方法:选取100例急性心肌梗死患者,时间为2017年2月-2019年2月,将100例急性心肌梗死患者分为心衰组(n=50)与无心衰组(n=50),另选取50例健康体检者作为对照组。对其血清甲状腺激素与外周血白细胞计数进行检测,分析血清甲状腺激素、外周血白细胞计数与急性心肌梗死心力衰竭严重程度的关系。结果:心衰组FT3低于非心衰组及对照组,且非心衰组低于对照组,差异有统计学意义(P<0.05)。心衰组WBC高于非心衰组及对照组,且非心衰组高于对照组,差异有统计学意义(P<0.05)。心衰Ⅳ级患者FT3低于Ⅲ级及Ⅱ级患者,且Ⅲ级患者低于Ⅱ级患者,差异有统计学意义(P<0.05)。心衰Ⅳ级患者WBC高于Ⅲ级及Ⅱ级患者,且Ⅲ级患者高于Ⅱ级患者,差异有统计学意义(P<0.05)。急性心肌梗死心力衰竭严重程度与FT3呈负相关,与WBC呈正相关。结论:急性心肌梗死心力衰竭严重程度与FT3呈负相关,与WBC呈正相关,可在发病早期通过外周血白细胞计数及血清甲状腺激素水平对急性心肌梗死心力衰竭患者进行识别,通过有效干预为疾病的治疗提供帮助。
Objective:To investigate the role of serum thyroid hormone and peripheral blood leukocyte count in the assessment of the severity of heart failure in acute myocardial infarction.Method:A total of 100 patients with acute myocardial infarction were selected from February 2017 to February 2019.The 100 patients with acute myocardial infarction were divided into the heart failure group (n=50) and the non-heart failure group (n=50).Another 50 healthy persons were selected as the control group.The serum thyroid hormone and peripheral blood leukocyte count were detected.The relationship between serum thyroid hormone and peripheral blood leukocyte count and severity of heart failure in acute myocardial infarction were analyzed.Result:The FT3 of the heart failure group was lower than that of the non-heart failure group and the control group,and the non-heart failure group was lower than the control group,and the differences were statistically significant (P<0.05).The WBC of the heart failure group was higher than that of the non-heart failure group and the control group,and the non-heart failure group was higher than the control group,and the differences were statistically significant (P<0.05).The FT3 of patients with grade Ⅳ heart failure was lower than that of patients with grade Ⅲ and grade Ⅱ,and that of patients with grade Ⅲ was lower than that of patients with grade Ⅱ,the differences were statistically significant (P<0.05).The WBC of patients with grade Ⅳ heart failure was higher than that of patients with grade Ⅲ and grade Ⅱ,and that of patients with grade Ⅲ was higher than that of patients with grade Ⅱ,the differences were statistically significant (P<0.05).The severity of heart failure in acute myocardial infarction was negatively correlated with FT3 and positively correlated with WBC.Conclusion:The severity of heart failure in acute myocardial infarction is negatively correlated with FT3 and positively correlated with WBC,which can be identified in patients with heart failure in acute myocardial infarction at the early stage of the disease by means of peripheral blood leukocyte count and serum thyroid hormone level,and provide assistance for the treatment of the disease through effective intervention.
