集束化护理措施在无肝素连续性血液净化治疗中的应用

Application of Cluster Nursing Measures in Continuous Blood Purification without Heparin

目的:探讨集束化护理措施在无肝素连续性血液净化治疗中的应用价值。方法:选取2017年1月-2019年12月笔者所在医院肾病科接收的90例无肝素连续性血液净化治疗患者作为研究对象,按照随机数字表法分为常规组与观察组,每组45例。常规组在治疗期间接受常规护理,观察组在治疗期间接受集束化护理,观察两组治疗时间、目标完成时间、滤器凝血发生率、护理依从性及护理满意度。结果:观察组治疗时间为(15.26±7.22)h,长于常规组的(7.03±3.64)h,差异有统计学意义(P<0.05);观察组目标完成时间比为(64.34±0.18)%,高于常规组的(30.05±0.15)%,差异有统计学意义(P<0.05);观察组滤器凝血发生率(2.22%)明显低于常规组(15.56%),差异有统计学意义(P<0.05);观察组护理依从性(95.55%)明显高于常规组(80.00%),差异有统计学意义(P<0.05);观察组护理总满意度(88.89%)明显高于常规组(64.44%),差异有统计学意义(P<0.05)。结论:集束化护理措施在无肝素连续性血液净化治疗中的应用效果显著,不仅能够有效延长患者的治疗时间,还能够显著减少凝血情况,可以考虑在临床中广泛推广。

Objective:To explore the application value of cluster nursing measures in continuous blood purification without heparin.Method:From January 2017 to December 2019,90 patients who received continuous blood purification without heparin in nephrology department of our hospital were selected as research objects.The patients were divided into the routine group and the observation group according to the random number table method,45 cases in each group.The routine group received routine nursing during treatment,while the observation group received cluster nursing during treatment.The treatment time,target completion time,incidence of filter coagulation,nursing compliance and nursing satisfaction of the two groups were observed.Result:The treatment time was (15.26±7.22) hours in the observation group,which was longer than (7.03±3.64) h of the routine group,and the difference was statistically significant (P<0.05).The goal completion time ratio was (64.34±0.18)% in the observation group,which was higher than (30.05±0.15)% of the routine group,and the difference was statistically significant (P<0.05).The incidence of filter coagulation in the observation group was 2.22%,which was significantly lower than 15.56% of the routine group,and the difference was statistically significant (P<0.05).The nursing compliance of the observation group was 95.55%,which was significantly higher than 80.00% of the routine group,and the difference was statistically significant (P<0.05).The total nursing satisfaction of the observation group was 88.89%,which was significantly higher than 64.44% of the routine group,and the difference was statistically significant (P<0.05).Conclusion:The application effect of cluster nursing measures in continuous blood purification without heparin is significant.It can not only effectively prolong the treatment time of patients,but also reduce the coagulation situation.Therefore,it can be widely promoted in clinical practice.
