TPRK术中应用眼球自旋控制功能矫正近视合并中高度散光的临床研究

摘要 目的:探讨在经上皮准分子激光角膜切削术(trans-epithelial photorefractive keratectomy,TPRK)中应用眼球自旋控制功能矫正近视合并中高度散光的临床效果。方法:回顾性分析近视合并中高度散光(-1.00^-4.00 D)患者79例(156眼)临床资料,均行TPRK术,并于术中应用阿玛仕准分子激光手术系统的眼球自旋控制功能。术后6个月,分析患者裸眼视力、残余散光和满意度情况。结果:术后6个月,裸眼视力与术前最佳矫正视力一致或较术前最佳矫正视力提高1行及以上的有147眼(94.23%);术后裸眼视力与术前最佳矫正视力比较,差异无统计学意义(P>0.05);术后6个月,残余散光≤1.0 D的有152眼(97.44%);术后残余散光显著低于术前,差异有统计学意义(P<0.05);患者总满意度为98.73%。结论:在TPRK术中采用阿玛仕准分子激光手术系统的眼球自旋控制功能矫正近视合并中高度散光安全有效,具有术后视觉质量高、残余散光少、患者满意度高的优势,值得在临床中推广。 Objective:To investigate the clinical effect of using the cyclotron control in the treatment of myopia combined with moderate and high astigmatism in trans-epithelial photorefractive keratectomy(TPRK).Method:The clinical data of 79 cases(156 eyes)of myopia combined with moderate and high astigmatism(-1.00 to-4.00 D)were retrospectively analyzed.All of them underwent TPRK,and the cyclotron control of the Amaris excimer laser system was applied during the operation.The visual acuity,residual astigmatism and satisfaction at 6 months after operation were analyzed.Result:At 6 months after operation,there were 147 eyes(94.23%)whose visual acuity was consistent with or improved by 1 line or more than the preoperative best corrected visual acuity.Postoperative visual acuity and preoperative best corrected visual acuity was compared,and the difference was not statistically significant(P>0.05).At 6 months after operation,there were 152 eyes(97.44%)with residual astigmatism≤1.0 D.Postoperative residual astigmatism was significantly lower than that before operation,and the difference was statistically significant(P<0.05).Total satisfaction of patients was 98.73%.Conclusion:It is safe and effective to correct myopia combined with moderate and high astigmatism by using the cyclotorsion control of the Amaris excimer laser system in TPRK.It has the advantages of high visual quality,less residual astigmatism and high satisfaction of patients,which is worthy of clinical promotion.
