256-SCTA诊断颅内动脉瘤的临床价值探析 被引量:1

摘要 目的探讨256层螺旋CT血管造影(256-SCTA)诊断颅内动脉瘤的临床价值。方法选择医院2015年8月至2018年5月收治的怀疑颅内动脉瘤患者78例,对其临床资料进行回顾性分析,所有患者均进行256-SCTA与数字减影血管造影(DSA)检查,将256-SCTA与DSA检查与手术结果进行比较,对比256-SCTA与DSA诊断的敏感度、特异度、准确性及Kappa值,并采用ROC曲线分析256-SCTA与DSA在评估颅内动脉瘤中的价值。结果 256-SCTA诊断颅内动脉瘤的敏感度为96.92%,特异度为87.50%,准确度为95.89%,Kappa值为0.800;DSA诊断颅内动脉瘤的敏感度为92.31%,特异度为75.00%,准确度为90.41%,Kappa值为0.578。256-SCTA诊断与手术结果的一致性明显高于DSA(P<0.05)。ROC曲线分析显示,256-SCTA评估颅内动脉瘤的敏感性高于DSA(P<0.05)。结论 256-SCTA可真实反映脑动脉瘤影像,与手术检查结果保持较高的一致性,具有较高的预测价值。 Objective To explore the clinical value of 256-slice spiral CT angiography (256-SCTA) in the diagnosis of intracranial aneurysms.Methods A total of 78 patients suspected as intracranial aneurysms admitted to the hospital from August 2015 to May 2018 were selected.The clinical data were retrospectively analyzed.All patients were given 256-SCTA and digital subtraction angiography (DSA).The results of 256-SCTA and DSA examinations were compared with surgical results,and the sensitivity,specificity,accuracy and Kappa value of 256-SCTA and DSA diagnosis were compared,and the ROC curve was used to analyze the value of 256-SCTA and DSA in the evaluation of intracranial aneurysms.Results The sensitivity,specificity,accuracy and Kappa value of 256-SCTA in the diagnosis of intracranial aneurysms were 96.92%,87.50%,95.89% and 0.800.The sensitivity,specificity,accuracy and Kappa value of DSA in the diagnosis of intracranial aneurysms were 92.31%.75.00%,90.41% and 0.578.The consistency of 256-SCTA diagnosis with surgical results was significantly higher than those of DSA (P<0.05).ROC curve analysis showed that 256-SCTA was more sensitive than DSA in evaluating intracranial aneurysms (P<0.05).Conclusion 256-SCTA can accurately reflect the image of cerebral aneurysm,and maintain high consistency with the results of surgical examination,and it has high predictive value.
