摘要 目的:评估机械性踝关节不稳(mechanical ankle instability,MAI)患者是否存在动态稳定性下降的缺陷,为此类患者的治疗策略提供依据。方法:42名测试对象纳入本研究,其中23名为机械性踝关节不稳患者(A组),19名健康对照人群(B组)。每名测试对象均在平衡测试训练仪(Neuro Com Balance Manager)上进行运动控制测试(motor control test,MCT),评估其在突发外源干扰时迅速恢复平衡的能力。采用重心对称评分(weight symmetry,WS)和潜伏期评分(latency score,LS)对受试者动态稳定能力进行评估。结果:两组间在性别、年龄、身高、体重方面差异无统计学意义(P>0.05)。MAI患者在所有后向摆动及前向小幅摆动WS均高于对照组,差异具有统计学意义(P<0.05),表明MAI患者利用患侧肢体来维持重心的能力明显下降。两组间LS在前、后摆动状态下差异均未见统计学意义。组内比较,患侧与健侧在前、后摆动状态下LS差异也未见统计学意义(P>0.05)。结论:机械性踝关节不稳患者存在动态平衡功能异常,利用患侧肢体来维持身体重心的能力下降。在临床治疗中,应当注意这些异常,并进行针对性康复训练。 Objective To evaluate the dynamic stability of patients with mechanical ankle instability(MAI),so as to provide a scientific support for the treating strategy.Methods Forty two participants were recruited in this study,including 23 patients with MAI(group A)and 19 healthy control people.All subjects underwent the motor control test(MCT)on a balance testing instrument(Neuro Com Balance Manager system).The MCT was used to measure the subjects’ability to recover after an unexpected external disturbance.The dynamic stability evaluated using the weight symmetry(WS)and latency score(LS).Results No significant difference was found in the sex,age,height and weight between the two groups(P>0.05).The WS of small,medium and large posterior translations and small anterior translation of MAI group were significantly higher than the control group(P<0.05),which indicated the unsymmetry use of bilateral limbs in keeping balance of MAI patients.No significant difference was found in LS between the two groups or within the groups.Conclusion The dynamic stability of the MAI patients is abnormal,as the injured leg has decreased ability to keep balance.In the clinical practice,such abnormalities should be noticed,and specific rehabilitation treatment should be provided.
