1例再障且头孢曲松过敏患儿使用万古霉素的药学监护

Pharmaceutical Care for a Child with Aplastic Anemia and Allergic to Ceftriaxone Treated with Vancomycin

临床药师对1例明确蜂窝组织炎,有严重再生障碍性贫血基础疾病,且近期有头孢曲松过敏性休克史的患儿实施药学监护,关注患者用药的安全性、疗效、不良反应等,利用血药浓度监测协助医师制定用药方案,调整万古霉素的用药方案,保障患者药物治疗的安全、有效。本案例为临床药师参与儿童使用万古霉素的药学监护提供参考。

Clinical pharmacists performed pharmacological monitoring on a patient with cellulitis,who had severe aplastic anemia and a recent history of ceftriaxone anaphylactic shock.Clinical pharmacists focused on the safety,efficacy and adverse reactions of medicines used in the patient,and assisted doctors in making treatment regimen and adjusting the dose of vancomycin based on the therapeutic drug monitoring(TDM)to guarantee the patient obtaining safe and effective drugs during the treatment.The case provided reference for clinical pharmacists to perform pharmaceutical care for children treated with vancomycin.
