摘要
目的探讨血清超敏C-反应蛋白(hs-CRP)在儿童支气管哮喘合并过敏性鼻炎患者中的变化及临床意义。方法选取单纯过敏性鼻炎(AR组)患儿100例、单纯性支气管哮喘(BA组)患儿100例及支气管哮喘合并过敏性鼻炎(BA+AR组)患儿100例,另选取同期至本院健康体检的儿童100例为对照组。比较4组血清hs-CRP水平的差异,分析血清hs-CRP水平与肺功能、IgE等指标的相关性。结果与对照组相比,AR组、BA组、BA+AR组IgE、血清hs-CRP水平、嗜酸性粒细胞计数、FeNO均显著升高,而FEV1占预计值百分比均显著降低,差异均有统计学意义(P <0.05);BA+AR组IgE、血清hs-CRP水平、嗜酸性粒细胞计数、Fe NO均显著高于AR组和BA组,而FEV1占预计值百分比显著低于AR组和BA组,差异均有统计学意义(P <0.05);Pearson相关分析提示,在支气管哮喘合并过敏性鼻炎患儿中,血清hs-CRP水平与IgE、嗜酸性粒细胞计数和FeNO呈正相关(r值分别为0.211、0.206、0.233,P <0.05),与FEV1占预计值百分比呈负相关(r=-0.242,P <0.001);Logistic回归分析提示,血清hs-CRP水平是支气管哮喘合并过敏性鼻炎的影响因素(P <0.05)。结论支气管哮喘合并过敏性鼻炎患儿血清hs-CRP水平明显升高,并且血清hs-CRP水平是支气管哮喘合并过敏性鼻炎的独立危险因素。
Objective To investigate the changes of serum hypersensitive C-reactive protein(hs-CRP)level and its clinical significance in children with bronchial asthma combined with allergic rhinitis.Methods A total of 100 children with allergic rhinitis(the AR group),100 children with bronchial asthma(the BA group),100 children with bronchial asthma combined with allergic rhinitis(the BA+AR group)and 100 healthy children were enrolled in our study.The differences of serum hs-CRP levels in the four groups were compared and the relationship between serum hs-CRP levels and clinical biomarkers was analyzed.Results Compared with the control group,children in the AR group,the BA group and the BA+AR group had higher IgE,serum hs-CRP levels,eosnophils counts and FeNO while lower FEV1%,with the differences statistically significant(P<0.05).Children in the BA+AR group had higher IgE,serum hs-CRP levels,eosnophils counts and FeNO while lower FEV1%than the BA group and the AR group,with the differences statistically significant(P<0.05).In children with BA+AR,serum hs-CRP level were significantly positive correlated with IgE,eosnophils counts,and FeNO(r=0.211,r=0.206,r=0.233,P<0.05),while significantly negative correlated with FEV1%(r=-0.242,P<0.001).Logistic analysis showed that serum hs-CRP level was an independent risk factor for the BA+AR(P<0.05).Conclusion Children in the BA+AR group has higher serum hs-CRP level which is an independent risk factor for the BA+AR.
