中医辨证施护在改善慢性心力衰竭患者心功能及提高患者生活质量中的作用

The role of TCM syndrome differentiation and nursing in improving cardiac function and quality of life of patients with chronic heart failure

目的探讨中医辨证施护在改善慢性心力衰竭(CHF)患者心功能及提高患者生活质量中的作用。方法选取该院2016年7月至2017年10月收治的CHF患者88例,按随机数字表法分为对照组和观察组,各44例。对照组予以常规护理干预,观察组在对照组的基础上实施中医辨证施护干预。统计比较两组患者干预前后的心功能分级、左心室射血分数(LVEF)、6 min步行距离、明尼苏达心力衰竭生活质量调查问卷(MLHFQ)评分等指标情况,并比较护理效果及护理满意度。结果观察组护理总有效率(90.91%)明显高于对照组(70.45%);干预后观察组的心功能分级明显优于对照组;干预后观察组的LVEF值、6 min步行距离均高于对照组;干预后两组各证型MLHFQ评分较干预前均降低,且观察组较对照组低;观察组的护理总满意率(93.18%)明显高于对照组(72.73%),以上差异均有统计学意义(均P<0.05)。结论中医辨证施护应用于CHF,可改善患者的心功能,提高患者的生活质量及对护理的满意度,护理效果显著。

Objective To explore the role of TCM syndrome differentiation in improving cardiac function and quality of life of patients with chronic heart failure(CHF).Methods A total of 88 CHF patients admitted to the hospital from July 2016 to October 2017 were selected and divided into control group and observation group according to the random number table method,with 44 patients in each group.The control group received routine nursing intervention,and the observation group carried out TCM syndrome differentiation nursing intervention on the basis of the control group.Cardiac function grade,left ventricular ejection fraction(LVEF),6 minutes walking distance,Minnesota Heart Failure Quality of Life Questionnaire(MLHFQ)score and other indicators were statistically compared between the two groups before and after intervention,and nursing effect and nursing satisfaction were compared.Results The total effective rate of treatment in observation group(90.91%)was higher than that in the control group(70.45%).The classification of cardiac function in the observation group was better than that of the control group after intervention,and the difference was statistically significant(P<0.05).After intervention,the LVEF value and 6 minutes walking distance in the observation group were higher than those in the control group.After intervention,the MLHFQ scores of the two groups were lower than those before intervention,and the observation group was lower than the control group.The total satisfaction rate of the observation group(93.18%)was higher than that of the control group(72.73%).And all the above differences were statistically significant(P<0.05).Conclusions The application of TCM syndrome differentiation and nursing in CHF can improve the cardiac function,and improve the quality of life and nursing satisfaction of patients.Besides,the nursing effect is remarkable.
