乳腺癌区域淋巴结转移与超声声像特征关系的回顾性分析

摘要 目的探讨乳腺癌区域淋巴结转移与超声声像特征关系。方法回顾性收集2010年1月-2019年12月于北票市中心医院超声科进行乳腺超声检查并最终经病理证实为乳腺癌的患者462例,依据术后病理是否存在淋巴结转移分为转移组202例和非转移组260例。收集两组患者的一般资料和肿瘤超声影像学特征,对导致区域淋巴结转移的危险因素进行单因素和多因素分析。结果单因素分析结果显示,肿瘤直径.病理类型、毛刺征.钙化和血流分级是出现区域淋巴结转移的危险因素(P<0.05)。多因素分析结果显示,肿瘤直径、毛刺征.钙化和血流分级是出现区域淋巴结转移的危险因素(P<0.05)。结论对于乳腺癌患者,如果超声所见肿瘤直径较大、存在毛刺征和钙化以及血流分级为Ⅲ~Ⅳ级,在进行区域淋巴结超声扫查时更应警惕是否存在淋巴结转移。 Objeetive To investigate the relationship between lymph node metastasis and ultrasound features in breast cancer.Methods A total of 462 patients with breast cancer who were examined by ultrasonography in our hospital from January 2010 to December 2019 and finally confirmed by pathology were retrospectively collected.According to whether there was lymph node metastasis after operation,they were divided into two groups:202 cases of metastasis group and 260 cases of non metastasis group.To ollect the general data and the ultrasonic imaging features of patients in the two groups and analyze the risk factors of regional lymph node metastasis.Results Univariate analysis showed that tumor diameter,pathological type,spicule sign,calcification and blood flow classification were the risk factors of regional lymph node metastasis(P<0.05).The multivariate analysis showed that tumor diameter,spicule sign,calcifcation and blood flow classifcation were the risk factors of regional lymph node metastasis(P<0.05).Conclusion For breast cancer patients,if the tumor diameter is large,there are spicule sign and calcification,and the blood flow classification is II-IV,we should pay more attention to whether there is lymph node metastasis in the regional lymph node ultrasound scanning.
