氢氧化钙糊剂联合根管治疗术对牙髓炎的疗效

Effect Observation of Calcium Hydroxide Paste Plus Root Canal Therapy on Treatment of Pulpitis or Apicitis

目的探究牙髓炎或根尖炎应用根管填充剂氢氧化钙糊剂联合根管治疗手术的临床疗效,并对患者治疗期间血清炎性因子及牙周指数进行分析。方法选取本院2018年1月—2019年1月收治牙髓炎或根尖炎患者55例作为研究对象,根据平行对比法设定组别,L组实施根管填充剂氢氧化钙糊剂联合根管治疗手术(n=27),H组实施氧化锌碘仿丁香油黏固粉联合根管治疗手术(n=28),观察L组与D组治疗期间的牙周指数及血清炎性因子,并采用统计学软件对临床疗效进行分析。结果治疗后,L组血清中IL-6、IL-8、CRP、TNF-α均优于H组对比存在统计学意义(P <0.05);治疗第6周后进行复查,L组牙周指数PIL、BI、PD及CAL与H组比较差异有统计学意义(P<0.05)。结论氢氧化钙糊剂较氧化锌碘仿丁香油黏固粉联合根管治疗术治疗牙髓牙及根尖周炎的临床疗效显著,且患者血清炎性细胞因子和牙周指数均得到改善。

Objective To explore the clinical effect of root canal filler with calcium hydroxide paste plus root canal therapy on treatment of pulpitis or apicitis and analyze patient’s serum inflammatory factors and periodontal index. Methods From January 2018 to January 2019, 55 pulpitis or apicitis patients(2018.01-2019.01) were selected and assigned to L group(n=27) and H group(n=28) according to parallel contrast method. L group took the root canal filler with calcium hydroxide paste and root canal therapy;H group took eugenol/zinc oxide cement and root canal therapy. The periodontal index, serum inflammatory factors and curative effect were analyzed. Results Serum IL-6(interleukin-6), IL-8(interleukin-8), CRP(C-reactive protein) and TNF-α(tumor necrosis factor-α) levels in L group were better than those of H group, with statistically significant difference(P<0.05);after 6 w of review, PLI(plaque index), BI(bleeding index), PD(probing depth) and CAL(clinical attachment level) between L group and H group showed statistically significant difference(P<0.05). Conclusion Calcium hydroxide paste combined with root canal therapy is proven to be more effective, which can improve patient’s serum inflammatory factors and periodontal index.
