摘要
目的比较高速涡轮钻法和锤凿法在拔除下颌水平阻生牙的临床效果,评价高速涡轮钻法的优越性。方法选取2017年12月—2018年12月本院口腔科收治的102例下颌水平阻生牙患者作为研究对象,随机分为对照组与治疗组两组,每组各51例。治疗组应用高速涡轮钻拔牙法,对照组应用传统锤凿拔牙法。比较两组阻生牙拔除时间、拔牙窝完整性、张口受限程度、疼痛程度及术后不良反应发生率的差异。结果治疗组拔牙时间显著低于对照组(P<0.05);治疗组拔牙窝完整性与对照组存在显著差异(P<0.05);治疗组张口受限程度、疼痛评分均显著低于对照组(P<0.05),术后不良反应发生率亦明显低于对照组(P<0.05)。结论在下颌水平阻生牙拔除中,使用高速涡轮钻微创拔牙法比锤凿拔牙法手术时间更短,术中、术后并发症发生率更低,适合在临床中推广使用。
Objective To compare the clinical effects of high speed turbine drilling and hammer chisel in extraction of horizontally impacted mandibular third molar,to evaluate the advantages of high speed turbine drilling method.Methods 102 patients with horizontally impacted mandibular third molar those admitted to the first hospital of Longhai were selected as study subjects and they were randomly divided into control group and treatment group,51 cases in each group.The treatment group was treated with high-speed turbo drill method and the control group was treated with traditional hammer-chisel extraction.To compare the difference of extraction time,integrity of extraction socket,limitation of mouth opening,pain degree and postoperative adverse reaction rate between two groups.Results The extraction time in the treatment group was significantly lower than that in the control group(P<0.05);the intact extraction socket in the treatment group was significantly different from that in the control group(P<0.05);the degree of mouth opening restriction and pain score in the treatment group were significantly lower than those in the control group(P<0.05),and the incidence of adverse reactions after operation was also significantly lower than that in the control group(P<0.05).Conclusions Minimally invasive extraction of high-speed turbine drilling is better than the hammer chisel tooth extraction method in extraction of horizontally impacted mandibular third molar,with shorter operation time and fewer intraoperative and postoperative complications,which is suitable for clinical application.
