血脂水平及炎症指标在血液透析患者中的表达及意义

Expression of serum lipid level and inflammatory markers in patients receiving hemodialysis and the significance

目的探讨维持性血液透析(MHD)患者血脂异常与炎症标记物的关系。方法选取2014-2016年本院收治的终末期肾脏病(ESRD)中接受维持性血液透析(MHD)的患者40例及同期的健康志愿者40例为研究对象,接受维持性血液透析(MHD)的患者为实验组,同期的健康志愿者为对照组。测定两组人群的血脂水平、血超敏C反应蛋白(hs-CRP)和睾酮水平,并进行统计学分析。结果 MHD患者的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)水平均显著低于健康正常人,而脂蛋白A水平高于健康正常人,差异具有统计学意义(P<0.05)。同时,MHD患者的载脂蛋白A1、载脂蛋白B水平显著降低(P<0.01),而超敏C反应蛋白升高(P<0.01),男性患者血清睾酮(T)平均水平显著降低,差异具有统计学意义(P<0.05)。结论 MHD患者血脂水平与炎症指标异常的程度与心血管的发病率有一定的关系。

Objective This study was conducted to explore the relationship between lipid abnormality and inflammatory markers in patients undergoing maintenance hemodialysis(MHD). Methods Totally 80 cases of study subjects including the patients with end-stage renal disease(ESRD) that receiving maintenance hemodialysis(MHD) in our hospital and healthy volunteers were enrolled in the research. The patients receiving MHD were in the experimental group and the volunteers were enrolled in the control group, forty cases in each group. Blood lipid levels, high sensitivity C-reactive protein(hs-CRP) and testosterone levels were measured in both groups. Statistical methods were then used to compare data of the two groups.Results Serum levels of total cholesterol(TC), triglyceride(TG),and low-density-lipoprotein(LDL) levels in MHD patients were significantly lower than those in healthy people, while the level of Apolipoprotein A was higher than the control group,and presented a statistically significant(P<0.05). Apolipoprotein A1 and apolipoprotein B levels in MHD patients were significantly lower than those of healthy individuals(P<0.01). However, inflammatory markers such as C-reactive protein(CRP) were significantly increased(P<0.01),and the average levels of serum testosterone in male patients were significantly decreased(P<0.05).Conclusions The serum lipid levels and inflammatory indices in MHD patients are related to the incidence of cardiovascular disease.
