摘要
目的探讨广东省清远地区肾结石患者的结石成分特点及复杂性和非复杂性肾结石成分差异及临床意义。方法回顾性分析2017年1月—2019年12月广州医科大学第六附属医院行结石成分分析的1280例肾结石患者的临床资料。复杂性和非复杂性肾结石分别463例和817例。分析结石成分分布差异。结果 1280例中,混合成分结石占多数,为80.0%,一水、二水草酸钙+碳酸磷灰石最多,占20.0%;单一成分结石以一水草酸钙结石最多,占12.9%。复杂组的碳酸磷灰石及磷酸铵镁结石比例明显升高,碳酸磷灰石(91.1%与47.7%,P<0.05)及磷酸铵镁结石(58.3%与11.8%,P<0.05)比例明显升高。结论广东省清远地区肾结石碳酸磷灰石及磷酸铵镁结石多见于复杂肾结石,草酸钙结石多见于非复杂肾结石,对清远地区肾结石治疗及预防复发有参考意义。
Objective To evaluate the constituents of renal calculus in Qingyuan, Guangdong, and evaluate the composition difference between patients with complex renal calculus and patients with noncomplex renal calculus and the clinical significance.Methods From January 2017 to December 2019,clinical data of 1280 kidney-stone patients those underwent composition analysis in the sixth affiliated hospital of Guangzhou Medical University was retrospectively analyzed.Among the 1280 cases, there were 463 complex renal calculus and 817 noncomplex ones.The distribution characteristics of renal calculus constituents in different groups were analyzed.Results Results:Among the 1280 cases, the mixed calculus were dominant in the kidney calculus(80.0%). In which, the most component was the calcium oxalate monohydrate or calcium oxalate dehydrate and carbonated apatite(20.0%).Among the pure-calculus,the most component was the calcium oxalate monohydrate(12.9%). The proportion of carbonated apatiterenal calculus and magnesium ammonium phosphatecalculus in the complex renal calculus group increased obviously. Carbonated apatiterenal calculus(91.1% and 47.7%,P<0.05) and magnesium ammonium phosphatecalculus(58.3% and 11.8%,P<0.05) were both raised.Conclusions The distribution of constituents of renal calculus in Qingyuan, Guangdong was different in patients of complex group and patients of noncomplex group. Carbonated apatite stones and magnesium ammonium phosphate calculus were more prevalent in complex renal calculus, while calcium oxalate calculus were more common in noncomplex renal calculus. It is of referential significance for the treatment and prevention of recurrence of calculus.
