基于RASS评分的身体约束决策表在ICU中的效果评价

目的探究100例应用基于RASS评分的身体约束决策表在重症监护病房(ICU)机械通气患者的应用效果。方法选取2018年9月—2019年1月福建省三甲某医院ICU病房收治的机械通气患者100例作为研究对象,随机分为实验组和对照组两组,每组各50例。实验组应用基于RASS评分的身体约束决策表指导进行约束评估,对照组根据临床经验进行约束评估,观察和比较两组机械通气患者非计划拔管发生率及身体约束护理合格率。结果实验组非计划拔管率显著低于对照组,实验组身体约束护理合格率明显高于对照组,实验组的非计划拔管率和身体约束合格率与对照组差别有统计学意义(P<0.05)。结论应用基于RASS评分的身体约束决策表能够显著降低ICU机械通气患者非计划性拔管的发生率和提高身体约束合格率,在ICU机械通气患者的约束评估中的应用效果良好,减少了身体约束带来的不良后果,护理人员容易掌握,是一种值得推广的护理手段。

Objective To explore the application effect of RASS score based body constraint decision table in 100 cases of patients underwent mechanical ventilation in intensive care unit(ICU).Methods From September 2018 to January 2019,a total of 100 cases of patients underwent mechanical ventilation in ICU of a grade 3 hospital of Fujian province were enrolled as study subjects,and they were randomly divided into experimental group and control group with 50 cases in each group.In the experimental group the constraints assessment was guided by RASS score based physical constraints decision table,and in the control group constraint evaluation was conducted according to the clinical experience.The incidence of unplanned extubation and nursing qualification rate of physical constraints of the patients with mechanical ventilation in both groups were observed and compared.Results The unplanned extubation rate in the experimental group was significantly lower than that in the control group,and the qualified rate of physical restraint nursing in the experimental group was significantly higher than that in the control group.The difference of the unplanned extubation rate and the qualified rate of physical restraint between the experimental group and the control group was statistically significant(P<0.05).Conclusions Application of physical restraint decision table based on RASS score could significantly reduce the incidence of unplanned extubation and improve the qualified rate of physical constraints of patients underwent mechanical ventilation in ICU.Its application effect in evaluation of constraints of patients with mechanical ventilation in the ICU is pretty good.The bad consequences followed physical constraints is reduced and it is easy to master by nurses,and it is a kind of care means worthy to be promoted.
