远程会诊系统在基层医院产科的应用研究

Study on Application of Teleconsultation System in Obstetrics Department of Primary Hospital

目的了解远程会诊系统在基层医院产科管理中的应用情况。方法针对绍兴市12家综合医院产科的远程会诊情况进行问卷调查,了解各级医院产科远程会诊系统的开展情况,并随机选取在远程会诊系统就诊的54例孕产妇作为研究对象,了解孕产妇对远程会诊的认知情况。结果三级医院开展远程会诊(80.00%)高于二级医院(14.29%),具备条件的三级医院占比(100.00%)高于二级医院(28.57%)(p<0.05)。三级医院孕产妇远程会诊的诊断符合率(94.12%)高于二级医院(85.00%)(p>0.05)。不同文化程度、职业和居住地的孕产妇对远程会诊的需求及认可度差异具有统计学意义(p<0.05)。其中以城市、高中以上学历和职员孕妇的需求性和认可度最高。结论综合医院产科开展远程会诊能提高就诊效率,满足孕产妇对围生期监护质量的要求,保障孕产妇及围产儿的生命安全,具有较好的社会和经济效益。

Objective To explore the application of remote medical consultation system in obstetric management of grassroots hospitals.Methods Questionnaire survey was conducted to investigate the status of remote medical consultation in obstetrics departments of 12 general hospitals in Shaoxing,to understand its application in obstetrics departments of hospitals of different levels.Fifty-four pregnant and lying-in women who sought medical services on remote medical consultation system were randomly selected as the subjects to understand their recognition of remote medical consultation.Results The proportion of the hospitals with remote medical consultation in tertiary hospitals(80.00%)was higher than that in secondary hospitals(14.29%)(p<0.05).The proportions of hospitals qualified for conducting remote medical consultation(100.00%)was higher than that in secondary hospitals(28.57%)(p<0.05).The consistent diagnosis rate of remote medical consultation in tertiary hospitals(94.12%)was higher than that of secondary hospitals(85.00%)(p<0.05).The demand and recognition of remote medical consultation was significantly different for the pregnant and lying-in women of different educational levels,occupations,and places of residence(p<0.05).The pregnant and lying-in women of urban area,educational level of high school and above,and employed status had the highest demand and recognition of remote medical consultation.Conclusion Conducting remote medical consultation in obstetrics departments of general hospitals can improve the efficiency of medical services,satisfy the demand for high care quality of perinatal period of pregnant and lying-in women,ensure the safety of pregnant and lying-in women and perinatal infants,and has high social and economic value.
