腹股沟疝采用腹膜外腹腔镜疝气修补术与传统手术治疗的效果研究

Study on the Effect of External Peritoneal Laparoscopic Hernia Repair and Traditional Surgery for Inguinal Hernia

目的:探讨传统手术与腹膜外腹腔镜疝气修补术治疗腹股沟疝效果。方法:将本院2016年1月~2019年6月的100例腹股沟疝患者,随机分组,传统手术组对于就诊的腹股沟疝患者给予传统手术,腹膜外腹腔镜疝气修补术组对于就诊的腹股沟疝患者开展腹膜外腹腔镜疝气修补术。结果:腹膜外腹腔镜疝气修补术组的手术成功率是100.00%(50例),传统手术组手术成功率是100.00%(50例),P>0.05。手术前两组QOL评分接近,P>0.05;手术后两组患者的生活质量均显著提高,且腹膜外腹腔镜疝气修补术组QOL评分高于传统手术组,P<0.05。腹膜外腹腔镜疝气修补术组腹股沟疝治疗时间、患者下床走路时间、胃肠道功能恢复时间、术后24h痛觉评分均低于传统手术组,手术出血量低于传统手术组,P<0.05。腹膜外腹腔镜疝气修补术组尿路感染出现率低于传统手术组,P<0.05。结论:传统手术与腹膜外腹腔镜疝气修补术治疗腹股沟疝效果均较好,其中,腹膜外腹腔镜疝气修补术可缩短操作的时间和康复的时间,且可减轻患者疼痛,减少并发症和出血。

Objective:To investigate the effect of traditional operation and extraperitoneal laparoscopic hernia repair for inguinal hernia.Methods:A total of 100 inguinal hernia patients in our hospital from January 2016 to June 2019 were randomly divided into the traditional surgery group and the external peritoneal laparoscopic hernia repair group.Results:The operative success rate of the extraperitoneal laparoscopic hernia repair group was 100.00%(50 cases),while that of the traditional surgery group was 100.00%(50 cases),P>0.05.Before operation,QOL scores of the two groups were close(P>0.05).After operation,the quality of life of both groups was significantly improved,and the QOL score of the extraperitoneal laparoscopic hernia repair group was higher than that of the traditional surgery group(P<0.05).The treatment time of inguinal hernia,the time of patients walking out of bed,the time of gastrointestinal function recovery,and the pain score 24 hours after operation in the extraperitoneal laparoscopic hernia repair group were all lower than those in the traditional surgery group,and the amount of bleeding was lower than that in the traditional surgery group(P<0.05).The incidence of urinary tract infection in the peritoneal laparoscopic hernia repair group was lower than that in the traditional surgery group(P<0.05).Conclusion:Both traditional surgery and extraperitoneal laparoscopic hernia repair are effective in the treatment of inguinal hernia.Among them,extraperitoneal laparoscopic hernia repair can shorten the time of operation and recovery,reduce pain,complications and bleeding.
