股骨头置换术在老年头下型股骨颈骨折患者中的临床应用

Clinical Application of Femoral Head Replacement in Elderly Patients with Sub-cephalic Femoral Neck Fracture

目的:讨论股骨头置换术在老年人头下型股骨颈骨折患者中的治疗效果。方法:回顾性分析2010年11月~2018年10月于本院骨科行手术治疗的56例头下型股骨颈骨折患者的相关资料。根据手术术式的不同分为实验组(31例):行股骨头置换术治疗;对照组(25例):行空心钉内固定术治疗。对比两种手术方法在治疗老年人头下型股骨颈骨折患者中,手术操作时间、术中出血量、术后卧床康复时间。结果:两组患者经不同的手术治疗方式,实验组在手术操作时间、术中出血量显著高于对照组,术后卧床康复时间则少于对照组,P<0.05,具有统计学意义。结论:在治疗老年人头下型股骨颈骨折中相较于空心钉内固定术,股骨头置换术的术中出血更多,手术时间更长,术后卧床康复时间更短。

Objective:To discuss the therapeutic effect of femoral head replacement in elderly patients with subcephalic femoral neck fracture.Methods:Retrospectively analyzed the relevant data of 56 patients with subcephalic femoral neck fractures who underwent surgical treatment in the orthopedics department of our hospital from November 2010 to October 2018.According to different surgical methods,they were divided into experimental group(31 cases):femoral head replacement treatment;control group(25 cases):hollow nail internal fixation.Comparing the two surgical methods in the treatment of elderly patients with femoral neck fractures,the operation time,intraoperative blood loss,and postoperative bed rehabilitation time.Results:The two groups of patients underwent different surgical treatments.The experimental group had significantly higher operation time and intraoperative blood loss than the control group,and the postoperative bed recovery time was less than the control group,P<0.05,which was statistically significant.Conclusion:Compared with hollow nail fixation,the femoral head replacement surgery has more bleeding during operation,longer operation time,and shorter postoperative bed rehabilitation time in the treatment of subcephalic femoral neck fractures in the elderly.
