机械振动排痰机在重症肺炎老年患者机械通气气道干预中的应用 被引量:5

摘要 目的:研究重症肺炎老年患者应用机械振动排痰进行气道干预中的临床效果。方法:选择沈阳市第七人民医院收治的重症肺炎患者40例作为本研究的临床样本,按照患者入科日期进行分组,2018年1月~2018年4月收治的21例患者为对照组,2018年5月~2018年7月收治的19例患者为观察组。两组患者在常规进行疾病治疗的基础上给予专科护理,对照组采用翻身、体位引流及传统的人工叩背排痰进行干预,观察组采用机械通气气道干预进行振动排痰护理,排痰前后进行血气分析生化检验,主要评价指标为血液酸碱度、动脉血氧分压以及动脉血二氧化碳分压等,并对患者的机械通气治疗时间、胸部X射线疾病影像改善时间、胸部听诊疾病症状改善时间进行评价。结果:干预前两组患者的血液酸碱度、动脉血氧分压以及动脉血二氧化碳分压等指标差异无统计学意义(P>0.05);干预后,观察组各项指标均优于对照组,差异有统计学意义(P<0.05)。干预后观察组患者机械通气治疗时间、胸部X射线疾病影像改善时间、胸部听诊疾病症状改善时间观察组明显低于对照组,差异有统计学意义(P<0.05)。结论:重症肺炎患者通过机械振动排痰机进行干预,显著提高了肺内痰液等分泌物的清除能力,改善了患者的肺部通气功能,对因分泌物堵塞呼吸道而导致的呼吸障碍具有良好临床效果。 Objective:To study the clinical effect of mechanical vibration sputum discharge in elderly patients with severe pneumonia for airway intervention.Methods:Forty patients with severe pneumonia admitted to the Seventh People’s Hospital of Shenyang City were selected as the clinical samples of this study,and the patients were grouped according to the date of admission.The 21 patients admitted between January 2018 and April 2018 were the control group.The 19 patients admitted from May 2018 to July 2018 were in the observation group.Two groups of patients were given special care based on routine disease treatment.The control group used rollover,postural drainage and traditional artificial percussion sputum for intervention.The observation group used mechanical ventilation airway intervention for vibration sputum care.Perform blood gas analysis and biochemical tests,the main evaluation indicators are blood pH,arterial blood oxygen partial pressure and arterial blood carbon dioxide partial pressure,etc.,and the patient’s mechanical ventilation treatment time,chest X-ray disease imaging improvement time,chest auscultation disease symptom improvement time Evaluation.Results:There were no significant differences in the indexes of blood pH,arterial blood oxygen partial pressure and arterial blood carbon dioxide partial pressure between the two groups before intervention(P>0.05);after intervention,all indexes in the observation group were better than those in the control group.It is statistically significant(P<0.05).After intervention,the time of mechanical ventilation treatment,the time of chest X-ray disease image improvement,and the time of chest auscultation disease symptom improvement in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Severe pneumonia patients intervene through mechanical vibration sputum discharge machine,which significantly improves the ability of lungs to remove sputum and other secretions,improves the patient’s lung ventilation function,and is good for respiratory disorders caused by secretions blocking the airway Clinical effect.
