在西北燥邪下从湿论治消渴病

Treatment of Diabetes Mellitus under Northwest Dryness Syndrome from the Dampness Theories

新疆地处西北,长期处于燥邪浸漫之中,而糖尿病患者多以湿邪为患,故新疆地区糖尿病患者多具有外燥内湿、燥湿相兼的体质特点,在西北燥邪下从湿论治消渴病可从法、因、证、治四点出发。西北外燥内逼、饮食失调、饮食不畅、脏腑失调皆可生湿,内易生湿留而致脾胃蕴湿证,西北干燥兼湿、燥湿相互转化、协同为病,以致消渴阴伤愈剧。分型论治可分为寒湿证与湿热证二者,治法均以健脾除湿为主,根据致病特点及病理因素转归结合临床症状表现,组方名为“消渴健脾汤”。但根据湿热病邪阻遏气机的病机特性发现,除湿不可过于温燥,否则易耗阴伤津,素体阴虚者多加注意,除此之外不可操之过急,更宜缓图之。在西北燥证视角下从湿论治消渴病,是结合地方治病特色基础对消渴病机的深刻把握,更是体现了三因制宜、辨证论治,故临床疗效甚佳。

Xinjiang is located in the Northwest of China.It has been suffering from Pathogenic Dampness for a long time,while most of the diabetic patients suffer from Northwest Dryness Syndrome.Therefore,most of the diabetic patients in Xinjiang have the physical characteristics of adjusting both dryness and dampness.Treatment of Diabetes Mellitus under Northwest Dryness Syndrome can be started from four points:methods,causes,syndromes and treatments.In Northwest China,dampness can be caused by external dryness and internal pressure,eating disorder,poor diet,and viscera disorder.Internal dampness is easy to be generated and left,which leads to the syndrome of spleen and stomach dampness.In Northwest China,dryness and dampness are transformed into disease,which leads to the severe injury of Diabetes Mellitus.Classification and treatment can be divided into cold dampness syndromes and damp heat syndromes.The treatment methods are mainly spleen invigorating and dehumidification.According to the treatment characteristics and pathological factors combined with clinical symptoms,the formula is named“XiaokeJianpi Decoction”.However,according to the pathogenesis characteristics of damp heat disease,dehumidification should not be too warm and dry,otherwise it is easy to consume Yin and injure body fluid,and more attention should be paid to those with deficiency of element Yin.In addition,it should not be carried out too quickly,and it is better to slow down.From the perspective of Northwest Dryness Syndrome,the treatment of diabetic disease from dampness theories is a deep grasp of the pathogenesis of diabetes combined with the characteristicsof local treatments.It also reflects the Three Categories of Etiological Factors System as well as Chinese Diagnostics,so it has tremendous clinical effects.
