吕培文教授托法在难愈性肉芽肿性乳腺炎中的应用

Application of Professor LvPeiwen′s Promoting Method in Refractory Granulomatous Mastitis

肉芽肿性乳腺炎临床治疗棘手,中医药在肉芽肿性乳腺炎的治疗中具有一定优势,但难愈性肉芽肿性乳腺炎常多种病变类型共存,给治疗带来更大难度。吕培文教授为首都国医名师,我国著名的中医外科专家。吕老擅用托法治疗难愈性肉芽肿性乳腺炎,尤其在红肿、窦道、肿物并存的复杂情况下传承和创造性使用缓托法,取得了良好的疗效。吕老不仅在托法治疗理论上有传承,选方用药亦独具特色。吕老还创造性地将托法用于外治,改变了既往诸多医家认为的托法为中医外科内治总则的观念。缓托法由吕老老师,燕京外科名家赵炳南、王玉章老师提出,吕老传承名家经验,并进一步发挥细化其理论内涵及适应证,用于难愈性肉芽肿性乳腺炎的治疗。文中就吕老治疗难愈性肉芽肿性乳腺炎使用托法的理论基础、具体选方、辨证用药特色等进行总结,并对托法指导外治的用药、缓托法在肉芽肿性乳腺炎应用的具体时机、方法等进行论述。

The clinical treatment of granulomatous mastitis is difficult.Traditional Chinese medicine has certain advantages in the treatment of this disease.However,there are many kinds of pathological changes coexisting in refractory granulomatous mastitis,which makes the treatment more difficult.Professor LvPeiwen is a famous Capital National Chinese medicine doctor and a famous expert in traditional Chinese surgery in China.Prof.Lv is good at using promoting method to treat the refractory granulomatous mastitis,especially in the complex situation of the coexistence of red swelling,sinuses and lumps,inheriting and creatively using the method of stable promoting methodwhich has achieved good results.Prof.Lv not only inherits the theory of promoting method,but also has unique characteristics in the selection of prescriptiondrugs.Prof.Lv also creatively used promoting method for external treatment,which changed the concept that promoting therapy as the general principle of internal treatment in traditional Chinese medicine surgery.The stable promoting therapy was proposed by Dr.Zhao Bingnan and Dr.Wang Yuzhang,who are Prof.lv′s teacher and very famous surgeons in Yanjing SugerySchool.Prof.Lv inherited the experience and further developed and specified its theoretical connotation and indications for the treatment of refractory granulomatous mastitis.In this essay,the theoretical basis,specific prescription,characteristics of syndrome differentiation and drug use of promoting method in the treatment of refractory granulomatous mastitis by Prof.lv were summarized.And the external drugs used by the guidance of promoting method,specific timing andtreatment of stable promoting method in granulomatous mastitis were discussed.
