督脉灸结合针刺治疗小儿寒性腹泻51例临床观察

Clinical observation on governor vessel moxibustion with acupuncture treatment of 51 cases of diarrhea in cold pattern in children

目的观察督脉灸结合针灸对小儿寒性腹泻的临床效果。方法选取2018年1月—2019年3月期间收治的小儿寒性腹泻患者102例,按随机数字表法分为对照组和治疗组,每组51例。对照组患者给予蒙脱石散治疗,治疗组在对照组的治疗基础上给予督脉灸结合针刺治疗。观察两组患者的临床疗效、临床症状变化、血清中炎性因子白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-9(Interleukin-9,IL-9)、超敏C反应白蛋白(High sensitive C reaction albumin,hs-CRP)水平变化及不良反应。结果治疗后对照组患者临床总有效率88.2%,治疗组患者临床总有效率96.8%,治疗组临床总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗组患者治疗后住院时间、腹痛停止时间、腹泻停止时间、大便质地恢复时间与对照组比较明显缩短,差异有统计学意义(P<0.05)。两组患者治疗后血清中hs-CRP、IL-6、IL-9含量均有不同程度降低,与治疗前比较,差异有统计学意义(P<0.05);且治疗组患者治疗后血清中hs-CRP、IL-6、IL-9水平明显降低,与对照组比较,差异有统计学意义(P<0.05)。治疗后两组患者均未出现便秘、恶心、呕吐症状。治疗组患者治疗过程中亦未出现晕针、滞针、弯针、烫伤等现象。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论督脉灸结合针灸治疗小儿寒性腹泻临床疗效显著,可有效改善患儿临床症状,抑制炎性反应,降低不良反应发生率,值得临床推广。

Objective To observe the clinical effect of governor vessel moxibustion with acupuncture treatment on diarrhea in cold pattern in children.Methods A total of 102 patients with cold diarrhea admitted to our hospital from January,2018 to March,2019 were randomly divided into the control group(51)and the treatment group(51),by using random number table.The control group were given montmorillonite powder,and the treatment group was treated with governor vessel moxibustion combined with acupuncture on the basis of the treatment in the control group.The clinical efficacy,clinical symptoms,serum inflammatory factor levels,such as interleukin-6(IL-6),interleukin-9(IL-9),high sensitive C reaction albumin(hs-CRP)and adverse events were observed.Results after treatment,the total clinical effective rate in the treatment group was 96.8%,which was significantly higher than that(88.2%)in the control group,with statistical significance(P<0.05).Compared with the control group,the time of hospital stay,abdominal pain disappearance time,diarrhea disappearance time and stool texture recovery time were significantly shortened,with statistical significance(P<0.05).After treatment,the serum levels of hs-CRP,IL-6,IL-9 decreased,with statistical significance(P<0.05);and the treatment group had larger reductions than the control group,with statistical significance(P<0.05).After treatment,there were no adverse events such as constipation,nausea,vomiting in both group.In the treatment group,no adverse events such as fainting during acupuncture,sticking of needles,curved needles,or scald were found.There was no significant difference between the two groups in the incidence of adverse events.Conclusion Governor vessel moxibustion combinedwith acupuncture is effective in the treatment of diarrhea in cold pattern in children,which can effectively improve the clinical symptoms,inhibit inflammatory reaction and reduce the incidence of adverse reactions,which is worthy of clinical promotion.
