针刺联合清宫正骨手法治疗颈源性头痛的临床研究

Clinical Study of Acupuncture Combined with Qinggong Bone Setting Manipulation for Cervical Headache

目的探讨针刺联合清宫正骨手法治疗颈源性头痛的临床疗效。方法选取2018年11月—2019年10月期间就诊的80例颈源性头痛患者,按照随机数字表法及患者的就诊次序随机分为治疗组和对照组各40例,治疗组采用针刺联合清宫正骨手法治疗,对照组采用单纯针刺治疗。两组患者均隔日治疗1次,每周治疗3次,4周为1疗程,共1个疗程。观察两组治疗前后头痛强度指数视觉模拟评分法(Visual Analogue Scale,VAS)值、颈椎功能障碍指数(Neck Disability Index,NDI)、每日头痛指数,并比较两组临床疗效。结果治疗4周后和12周后的随访时,两组VAS值、NDI值及每日头痛指数均较治疗前有明显降低,差异有统计学意义(P<0.01);且同时间节点两组组间比较,治疗组VAS值、NDI值及每日头痛指数降低更显著,差异有统计学意义(P<0.01)。治疗组总有效率95.00%(38/40),对照组总有效率80.00%(32/40),两组比较差异有统计学意义(P<0.05)。结论针刺联合清宫正骨手法能够有效降低和改善颈源性头痛患者的头痛强度指数VAS值、NDI值以及每日头痛指数。针刺联合清宫正骨手法治疗颈源性头痛的疗效优于单纯针刺治疗。

Objective To investigate the clinical efficacy of acupuncture combined with Qinggongbone-settingmanipulation on cervical headache.Methods According to the inclusion criteria,80 patients with cervical headache from November 2018 to October 2019 were selected and randomly divided into atreatment group and a control group,with 40 patients in each group.The treatment group was treated with acupuncture Combined with Qinggongbone-setting manipulation,the control group was treated with acupuncture alone.The two groups of patients were treated once every other day,3 times a week,4 weeks as a course of treatment,a total of 1 course of treatment.The visual analogue scale(VAS),cervical dysfunction index(NDI)and daily headache index were observed before and after treatment,and the clinical efficacy of the two groups were compared.Results During the follow-up after 4 weeks of treatment and 12 weeks after treatment,the VAS value,NDI value and daily headache index of the two groups were significantly lower than before treatment,and the difference was statistically significant(P<0.01).Compared between groups,the VAS value,NDI value and daily headache index of the treatment group decreased more significantly,and the difference was statistically significant(P<0.01).The total effective rate of the treatment group(38/40)was 95.00%,and the total effective rate of the control group(32/40)was 80.00%.The difference between the two groups was statistically significant(P<0.05).Conclusion Acupuncture combined with Qinggong bone-setting technique can effectively reduce and improve the headache intensity index VAS,cervical dysfunction index NDI and daily headache index of patients with cervical headache.Acupuncture combined with Qinggong bone-setting manipulation on cervical headache is more effective than acupuncture alone.
