头皮合并患耳完全撕脱伤的再植术治疗体会

The experience of replantation treatment of scalp laceration combined with ear tear injury

目的 探讨头皮合并患耳完全撕脱伤再植术治疗体会.方法 2012年3月-2019年5月,我院收治的7例头皮合并耳部完全撕脱伤患者,采用吻合患侧颞浅动静脉重建血供,术中彻底清创后,头皮原位缝合,术后常规三抗对症处理.结果 本组7例头皮合并患耳完全撕脱伤患者重建血供后,撕脱头皮并患耳全部存活,术后7-10d未发生动静脉危象,随访1-6个月,修复后患侧头皮感觉恢复好,毛发逐渐生长正常.结论 临床出现头皮合并耳部撕脱伤时,行撕脱头皮再植术是治疗全头皮撕脱伤最有效的方法 ,术前注意积极抗休克治疗,术中一期探查修复撕脱离断血管神经,术后头皮外观恢复,头发正常生长,效果满意.

Objective To investigate the clinical effect of scalp avulsion injury with only one superficial temporal artery and vein anastomosed. Methods From March 2012 to May 2019,7 cases of scalp combined with ear avulsion injury were treated in our hospital.Blood supply was reconstructed by superficial temporal and arteriovenous anastomosis on the affected side.After thorough debridement during the operation,the scalp was sutured in situ,and postoperative symptomatic treatment of the third antibody Results In this group of 7 patients with scalp combined with complete avulsion injury to the ear,after reconstruction of blood supply,there was no arteriovenous crisis 7-10 days after operation.The patients were followed up for 1-6 months,the scalp on the affected side felt better after repair,and the hair gradually grew to normal. Conclusion High-quality vascular anastomosis and vascular management are the most effective methods for the treatment of total scalp avulsion injury,successful microsurgical replantation of the total scalp avulsion is possible even only using a superficial temporal artery and a superficial temporal vein.This method has a reliable clinical effect.
