切开复位内固定与手法整复石膏外固定治疗旋后外旋Ⅱ、Ⅲ度踝关节骨折效果对比

Comparison of the effect of open reduction and internal fixation and manual reduction and gypsum external fixation on supination-eversion fractures of ankle with Ⅱ and Ⅲ degrees

目的 比较切开复位内固定与手法整复石膏外固定治疗旋后外旋Ⅱ、Ⅲ度踝关节骨折效果.方法 随机选取2014年6月-2018年6月我院旋后外旋Ⅱ、Ⅲ度踝关节骨折患者45例,随机分为切开复位内固定治疗组(内固定组,n=25)和手法整复石膏外固定治疗组(外固定组,n=20),统计分析两组患者的Mazur评分、住院时间、住院费用、满意度评分、临床疗效、并发症发生情况.结果 内固定组患者的Mazur、满意度评分均显著高于外固定组(P<0.05),住院时间显著短于外固定组(P<0.05),住院费用显著低于外固定组(P<0.05),关节功能恢复的优良率为92.0%(23/25)显著高于外固定组70.0%(14/20)(P<0.05).结论 切开复位内固定治疗旋后外旋Ⅱ、Ⅲ度踝关节骨折效果较手法整复石膏外固定好.

Objective To compare the effect of open reduction and internal fixation and manual reduction and gypsum external fixation on supination-eversion fractures of ankle with Ⅱ and Ⅲ degrees. Methods 45 patients with supinationeversion fractures of ankle with Ⅱ and Ⅲ degrees treated in our hospital from June 2014 to June 2018 were selected and randomly divided into open reduction and internal fixation treatment group(internal fixation group,n=25) and manual reduction and gypsum external fixation treatment group(external fixation group,n=20).The Mazur score,length of hospital stay,hospitalization cost,satisfaction score,clinical efficacy,and complications of the two groups were statistically analyzed. Results The Mazur score and satisfaction score of patients in the internal fixation group were significantly higher than those in the external fixation group(P < 0.05);The length of hospital stay was significantly shorter than that in the external fixation group(P < 0.05);The cost of hospitalization was less than that in the external fixation group(P < 0.05);The excellent and good rate of joint function recovery was 92.0%(23/25) in the internal fixation group,which was significantly higher than 70.0%(14/20) in the external fixation group(P < 0.05). Conclusion The effect of open reduction and internal fixation is better than manual reduction and gypsum external fixation in treating supination-eversion fractures of ankle with Ⅱ and Ⅲ degrees.
