采用宫腔镜治疗黏膜下子宫肌瘤的效果观察

Observation on the Treatment of Submucous Uterine Leiomyoma by Hysteroscopy

目的探讨采用宫腔镜治疗黏膜下子宫肌瘤的效果。方法纳入2016年1月—2018年9月荆州市第五人民医院妇产科接收治疗的100例黏膜下子宫肌瘤患者,将其结合手术情况进行分组,对照组的50名患者实施了开腹手术,观察组患者进行宫腔镜黏膜下子宫肌瘤切除术。分析手术失血水平、耗时、总住院日数、切口VAS评分;治疗前后患者子宫黏膜下子宫肌瘤症状及健康相关生活质量UFS-QOL问卷评分;渗血、发热和腰痛发病率。结果观察组子宫黏膜下子宫肌瘤症状及健康相关生活质量UFS-QOL问卷评分、手术失血水平、耗时、总住院日数、切口VAS评分低于对照组,渗血、发热和腰痛发病率少于对照组,P<0.05。结论黏膜下子宫肌瘤患者在手术方式的选择方面,相对于开腹手术而言,实施宫腔镜黏膜下子宫肌瘤切除术手术具有明显的优势,操作时间短且出血少,缩短康复时间,减少渗血、发热和腰痛发病率。

Objective To investigate the effect of hysteroscopy in the treatment of submucous uterine leiomyoma.Methods From January 2016 to September 2018,100 patients with submucosal uterine leiomyoma treated by the department of obstetrics and gynecology of Jingzhou Fifth People's Hospital were divided into two groups.50 patients in the control group underwent laparotomy.Patients in the observation group underwent hysteroscopic submucosal myomectomy.The level of blood loss,time consuming,total hospitalization days,incision VAS score,UFSQOL questionnaire score of submucous uterine myoma symptoms and health-related quality of life before and after treatment,and the incidence of bleeding,fever and low back pain were analyzed.Results The UFSQOL questionnaire scores of submucous uterine myoma symptoms and health-related quality of life,operative blood loss,time-consuming,total hospitalization days and incision VAS score in the observation group were lower than those in the control group,and the incidences of bleeding,fever and low back pain in the observation group were lower than those in the control group.Conclusion Compared with laparotomy,hysteroscopic submucosal myomectomy has obvious advantages in the choice of surgical methods for patients with submucosal uterine leiomyoma,with shorter operation time,less bleeding and shorter recovery time.Reduce the incidence of bleeding,fever and low back pain.
