无创呼吸机联合乌司他丁治疗急性呼吸窘迫综合征的效果研究 被引量:2

摘要 目的探讨无创呼吸机与乌司他丁联合用于急性呼吸窘迫综合征的中的临床效果。方法选取2017年1月—2018年6月科室收治的急性呼吸窘迫综合征患者的70例作为研究对象,在遵循伦理学并满足患者知情权的基础上以1:1的比例将对象分为试验组和对照组,对照组单用无创呼吸机治疗,试验组在此基础上联用乌司他丁,比较两组患者治疗后的免疫功能及肝肾功能。结果临床资料统计:试验组治疗后平均动脉压、心率、呼吸频率三项指标低于对照组,组间有统计学意义(P<0.05);试验组患者治疗后的AST、ALT、BUN、CR水平低于对照组,四指标组间均有统计学意义(P<0.05);试验组患者治疗后的血清IL-6、ICAM-1下降幅度甚于对照组(P<0.05);试验组患者的临床有效率、VAP发生率、病死率分别为94.29%、8.57%、2.86%,三项指标均优于对照组的77.14%、34.29%、5.71%,临床有效率和VAP发生率差异具有统计学意义(P<0.05)。结论无创呼吸机与乌司他丁联合治疗急性呼吸窘迫综合征效果确切。 Objective To explore the clinical effect of noninvasive ventilator combined with ulinastatin in the treatment of acute respiratory distress syndrome.Methods 70 patients with acute respiratory distress syndrome admitted to the Department from January 2017 to June 2018 were selected as the research objects.On the basis of following the ethics and meeting the patients'right to know,the subjects were divided into the experimental group and the control group in the form of 1:1.The control group was treated with non-invasive ventilator alone,and the experimental group was combined with Ulinastatin to compare the immune function of the two groups after treatment.And liver and kidney function.Results Clinical data statistics:after treatment,the average arterial pressure,heart rate and respiratory frequency of the experimental group were lower than those of the control group,with statistical significance(P<0.05);The ast,alt,bun and Cr levels of the experimental group were lower than those of the control group,with statistical significance(P<0.05);The serum IL-6 and ICAM-1 levels of the experimental group were lower than those of the control group(P<0.05);The clinical effective rate,VAP incidence and mortality of the experimental group were 94.29%,8.57%and 2.86%,respectively,which were superior to those of the control group77.14%,34.29%and 5.71%,the difference of clinical effective rate and incidence of VAP was statistically significant(P<0.05).Conclusion The combination of noninvasive ventilator and ulinastatin is effective in the treatment of acute respiratory distress syndrome.
