奥硝唑联合复方氯己定漱口对慢性牙周炎的作用

Effect of ornidazole combined with compound chlorhexidine gargle on chronic periodontitis

目的研究奥硝唑联合复方氯己定漱口方法对慢性牙周炎牙周指标的作用。方法选择2017年11月-2019年1月在中山市三乡医院口腔科就诊的135例慢性牙周炎患者为观察对象。按随机数字表法分为联合组、过氧化氢组、对照组各45例,各组均给予基础性牙周治疗1个月,而后联合组给予奥硝唑联合复方氯己定漱口护理,过氧化氢组给予过氧化氢擦拭护理,对照组给予生理盐水的漱口护理,比较3个月后各组的牙周指标、龈沟液(GCF)量和基质金属蛋白酶-3(MMP-3)水平。其中牙周指标包含探诊深度(PD)、附着丧失(AL)及菌斑指数(PLI)。结果联合组PD及PLI在护理1个月及3个月后均低于过氧化氢组和对照组;联合组在护理3个月后的AL水平低于过氧化氢组和对照组;联合组和过氧化氢组的PD及PLI在护理1个月后及3个月后的水平均低于护理前;联合组的AL在护理3个月后的水平低于护理前;联合组和过氧化氢组的GCF量和MMP-3水平在护理1个月后及3个月后明显低于护理前;联合组和过氧化氢组GCF量和MMP-3在护理1个月及3个月后的水平低于对照组,且联合组低于过氧化氢组,差异均有统计学意义(P<0.05)。结论应用奥硝唑联合复方氯己定漱口的护理方法对慢性牙周炎患者的牙周指标具有较好的改善作用,值得临床推广应用。

Objective To study the effect of ornidazole combined with compound chlorhexidine gargle on periodontal index of chronic periodontitis.Methods From November 2017 to January 2019,135 patients with chronic periodontitis treated in Stomatology Department of Sanxiang Hospital of Zhongshan City were selected as the observation objects.According to the random number table,the patients were divided into combined group,hydrogen peroxide group and control group,with 45 cases in each group.Each group was given basic periodontal treatment for 1 month,then the combined group was given ornidazole combined with compound chlorhexidine gargle nursing,hydrogen peroxide group was given hydrogen peroxide wipe nursing,control group was given normal saline gargle nursing,and three months later,the periodontal indexes of each group,the content of gingival crevicularfluid(GCF)and the level of matrix metalloprotease-3(MMP-3)were compared.Among them,periodontal indexes included probing depth(PD),attachment loss(AL)and plaque index(PLI).Results In the combined group,PD and PLI were lower than those of the hydrogen peroxide group and the control group after one month and three months of nursing;the level of AL in the combined group was lower than that in the hydrogen peroxide group and the control group;the level of PD and PLI in the combined group and the hydrogen peroxide group after one month and three months of nursing were lower than those before nursing;the level of AL in the combined group was lower than that after three months than before nursing;the levels of GCF and MMP-3 in the combined group and the hydrogen peroxide group after one month and three months of nursing were significantly lower than those before the nursing;the levels of GCF and MMP-3 in the combined group were lower than those in the control group after one month and three months of nursing,and the combined group was lower than that in hydrogen peroxide group;the differences of all above were statistically significant(P<0.05).Conclusion The nursing method of ornidazole combined with compound chlorhexidine gargle could improve the periodontal indexes of patients with chronic periodontitis,which was worthy of clinical promotion and application.
