卡前列素氨丁三醇联合卡贝缩宫素注射液对剖宫产术后止血效果及凝血功能的影响

The effect of carprostetine tromethamine combined with carbetocin injection on hemostatic effect and coagulation function after cesarean section

目的观察卡前列素氨丁三醇注射液(欣母沛)联合卡贝缩宫素注射液对剖宫产术后止血效果及凝血功能的影响。方法选取2017年1月-2018年1月浠水县散花镇卫生院行剖宫产术的产妇60例,采取随机数字表法分为观察组和对照组,每组30例。对照组采用卡贝缩宫素注射液治疗,观察组采用欣母沛联合卡贝缩宫素注射液治疗。比较2组术后止血效果,治疗前后凝血指标、临床指标,产后2、12 h出血量。结果观察组术后止血总有效率为100.00%,高于对照组的70.00%(χ^2=8.255,P<0.01);治疗后,2组纤维蛋白原水平高于治疗前,凝血酶原、凝血酶及活化部分凝血活酶时间均短于治疗前,且观察组优于对照组(P<0.01);治疗后,观察组平均尿量、出现低血压次数、平均止血时间及去氧肾上腺素水平均优于对照组(P<0.01);观察组产后2、12 h出血量均少于对照组(P<0.01)。结论欣母沛联合卡贝缩宫素注射液对剖宫产术后止血效果较好,可有效改善产妇凝血功能,减少产后2、12h出血量。

Objective Observe the effect of carprostetine tromethamine injection(Xinmupei)combined with carbetocin injection on the hemostatic effect and coagulation function after cesarean section.Methods From January 2017 to January2018,60 cases of parturient undergoing cesarean section in Sanhua Township Hospital of Xishui County were selected and divided into observation group and control group with 30 cases in each group.The control group was treated with carbetocin injection,and the observation group was treated with xinmupei combined with carbetocin injection.The postoperative hemostatic effects,coagulation indexes and clinical indexes before and after treatment were compared between the 2 groups,and the bleeding volume at 2 and 12 hours postpartum.Results The total effective rate of hemostasis was 100.00%in the observation group,which was higher than 70.00%in the control group(χ^2=8.255,P<0.01);After treatment,the level of fibrinogen in the two groups was higher than that before treatment,the time of prothrombin,thrombin and activated partial thromboplastin were shorter than that before treatment,and the observation group was better than that in the control group(P<0.01);After treatment,the average urine output,the number of hypotension,the average time to stop bleeding and the level of phenylephrine in the observation group were better than those in the control group(P<0.01).Conclusion Xinmupei combined with carbetocin injection has a good hemostatic effect after cesarean section,can effectively improve the maternal blood coagulation function,and reduce the amount of bleeding after 2 and 12 hours.
