温法的临床应用

Clinical Application of Warming Method

温法可用于治疗脾肾虚寒所致相关病证,如2型糖尿病、慢性口腔疾病、周围性面神经麻痹、多囊卵巢综合征、皮肤疾病、泌尿系结石等。2型糖尿病脾肾虚寒证,早期,脾阳亏虚为主,以附子理中汤加减治疗;日久肾阳亏虚,以金匮肾气丸加减治疗。慢性口腔疾病脾肾阳虚证,以温补脾肾,运化水湿为主,可用附子理中汤、半夏厚朴汤、平胃散,加肉桂、麻黄、苍术、茯苓、砂仁等。周围性面神经麻痹脾阳虚、寒湿内盛证,可选用麻黄附子细辛汤合平胃散加味。多囊卵巢综合征脾肾阳虚证,在苍附导痰汤基础上选用炮附子、干姜、吴茱萸、肉桂等。皮肤疾病为寒湿所致者,采用麻黄附子细辛汤、薏苡附子败酱散、四妙丸、五苓散、葛根汤等。泌尿系结石为脾肾阳虚、水液停聚下焦所致者,当先强健脾胃,待脾胃功能恢复后,再行补肾化石利湿治疗,可用附子理中汤或补中益气汤加减,脾虚症状改善后采用济生肾气丸、猪苓汤加大叶金钱草、鸡内金、炒王不留行、石韦等。

Warming method can be used to treat the related diseases and syndromes caused by spleen-kidney deficiency and cold,such as type 2 diabetes mellitus,chronic oral diseases,peripheral facial paralysis,polycystic ovary syndrome,skin diseases,urinary calculi,etc.For type 2 diabetes mellitus with spleen-kidney deficiency cold syndrome at early stage manifested as deficient spleen yang,Modified Aconite Center-Regulating Decoction can be used.For the disease with long-time deficient kidney yang,Modified Golden Cabinet′s Kidney Qi Pill can be used.For chronic oral diseases with spleen-kidney yang deficiency syndrome,the treatment should focus on warming and tonifying the spleen and kidney,transporting and transforming fluid retention,Aconite Center-Regulating Decoction,Pinellia and Officinal Magnolia Bark Decoction,Stomach-Calming Powder plus Rougui(Cortex Cinnamomi),Mahuang(Herba Ephedrae),Cangzhu(Rhizoma Atractylodis),Fuling(Poria),Sharen(Fructus Amomi),etc.can be used.For peripheral facial paralysis with deficient spleen yang and excessive cold and dampness syndrome,Ephedra and Aconite and Asarum Decoction combined with Supplemented Stomach-Calming Powder can be used.For polycystic ovary syndrome with spleen-kidney yang deficiency syndrome,Rhizoma Atractylodis and Rhizoma Cyperi Phlegm-Expelling Decoction plus Fuzi(Radix Aconiti Lateralis Preparata),Ganjiang(Rhizoma Zingiberis),Wuzhuyu(Fructus Evodiae),Rougui(Cortex Cinnamomi),etc.can be used.For skin diseases due to cold and dampness,Ephedra and Aconite and Asarum Decoction,Coix and Aconite and Patrinia Powder,Wonderfully Effective Four Pill,Five Substances Powder with Poria and Pueraria Decoction,etc.can be used.For urinary calculi due to spleen-kidney yang deficiency,fluid retention in lower energizer,the treatment should first focus on strengthening the spleen and stomach to recover their functions,then nourishing the kidney and resolving calculi and removing dampness.Aconite Center-Regulating Decoction or Modified Center-Supplementing and Qi-Boosting Decoction can be used.After improving spleen-deficiency symptoms,Life-Saving Kidney Qi Pill,Polyporus Decoction plus Jinqiancao(Herba Lysimachiae) with big leaves,Jineijin(Endothelium Corneum Gigeriae Galli),baked Wangbuliuxing(Semen Vaccariae),Shiwei(Folium Pyrrosiae),etc.can be used.
