急诊科护士创伤后应激障碍与生存质量及睡眠质量的相关性研究

Study on the correlation between post-traumatic stress disorder,quality of life and sleep quality of emergency department nurses

目的探讨三级综合性医院急诊科护士创伤后应激障碍(PTSD)与生存质量及睡眠质量的关系。方法于2018年4-8月,采用整群抽样法抽取无锡市及苏州市6所213名三级综合性医院急诊科护士,应用创伤后应激障碍自评量表(PTSD-SS)、生存质量评定量表简表(WHOQOL-BREF)、睡眠质量指数(PSQI),对6所三级综合性医院进行横断面研究。结果急诊护士PTSD检出率29.11%。急诊护士PTSD总分(46.67±7.27)分,其在年龄、工作年限、省级专科护士、职称、职务、夜班数量方面,差异有统计学意义(P<0.05);急诊科护士PTSD与生存质量各维度呈显著负相关,与睡眠质量呈显著正相关;回归分析显示,是否是省级专科护士、生理领域、心理领域、社会领域、环境领域、睡眠质量是影响急诊科护士PTSD的重要因素。结论急诊科护士PTSD、生存质量及睡眠质量状态需要改善。因此,通过急诊护士积极应对创伤事件,提高生存质量及睡眠质量,降低PTSD的水平。

Objective To study the correlation between post-traumatic stress disorder(PTSD)quality of life and sleep quality of emergency department nurses among tertiary comprehensive hospitals.Methods Total of 213 emergency department nurses from tertiary comprehensive hospitals in Wuxi and Suzhou city were selected by convenience sampling.General information questionnaire,post-traumatic stress disorder self-rating scale,WHOQOL-BREF and Pittsburgh sleep quality index were used to perform the cross sectional study.Results The score of 213 emergency department nurses’PTSD status was(46.67±7.27),and 29.11%nurses were PTSD.Emergency department nurses’PTSD was negatively related to the factors scores of quality of life,and positively related to sleep quality(P<0.05).There were significant differences on the scores of ages,seniority,specialist nurse,professional position,position and night shift.Multiple regression analysis results showed that specialist nurse,physiological aspect,psychological aspect,social aspect,environmental aspect and sleep quality affected the nurses’PTSD in department of emergency.Conclusions The level of PTSD,quality of life and sleep quality need to be improved among Emergency department nurses.Measures should be carried out to enhance the nursing staffs’quality of life and sleep quality,positively cope with traumatic stress disorder,so as to reduce the incidence of PTSD.
