摘要 目的探讨经尿道双极等离子电切术(PKRP)对老年良性前列腺增生症(BPH)患者尿动力学恢复及血清P物质(SP)、前列腺素E2(PGE2)、5-羟色胺(5-HT)水平的影响。方法选取长安医院116例老年BPH患者为研究对象,按治疗方案的不同将其分为PKRP组(n=59)、经尿道前列腺汽化电切术(TVP)组(n=57)。比较两组的治疗效果。结果PKRP组的尿管留置时间、膀胱冲洗时间、住院时间均短于TVP组,术中出血量少于TVP组(P<0.05)。PKRP组的并发症总发生率低于TVP组(P<0.05)。术后1、3 d,两组的血清SP、PGE2、5-HT水平均升高,但PKRP组低于TVP组(P<0.05)。术后3、6个月,两组的PVR均降低,BC、Qmax均升高,且PKRP组优于TVP组(P<0.05)。结论PKRP术治疗老年BPH患者可减轻手术创伤,降低并发症发生风险及血清SP、PGE2、5-HT水平,改善尿流动力学,促进患者恢复。 Objective To investigate the effects of transurethral bipolar plasmakinetic resection of the prostate(PKRP)on urodynamic recovery and serum substance P(SP),prostaglandin E2(PGE2)and 5-hydroxytryptamine(5-HT)levels in elderly patients with benign prostatic hyperplasia(BPH).Methods A total of 116 elderly patients with BPH in Chang’an hospital were selected as the study objects and divided into PKRP group(n=59)and transurethrl eletrovaporization of the prostate(TVP)group(n=57)by the different treatment scheme.The therapeutic effects of the two groups were compared.Results The indwelling time of urinary catheter,bladder irrigation time and hospitalization time of the PKRP group were shorter than those of the TVP group,and the intraoperative blood loss volume was less than that of the TVP group(P<0.05).The total incidence of complications in the PKRP group was lower than that in the TVP group(P<0.05).At 1 and 3 days after operation,the serum SP,PGE2 and 5-HT levels in the two groups increased,but those in the PKRP group were lower than the TVP group(P<0.05).At 3 and 6 months after operation,the PVR decreased in both groups,BC and Qmax increased,and those in the PKRP group were better than the TVP group(P<0.05).Conclusion PKRP in the treatment of elderly patients with BPH can reduce surgical trauma,the risk of complications and serum SP,PGE2,5-HT levels,which can improve urodynamics and promote patient recovery.
