新型冠状病毒肺炎的中医病因病机与治则治法探讨

提出新型冠状病毒肺炎的中医病名为"肺疫",病因为寒、湿、热、毒等不同属性病邪侵袭,初起核心病机是外邪郁肺困脾,病变过程存在热(火)、寒、湿、毒、痰、滞、结、瘀、燥、虚等病机变化,应给予清热、散寒、化湿、解毒、祛痰、导滞、攻结、化瘀、润燥、补虚等法治疗;初中期邪在卫气分,总以清解宣透卫气分邪毒为基本治则,慎用苦寒、补益之品,邪损肺络的重型、危重型应注重凉血散血方药的应用,而中医药及早介入治疗可有效防治该病。

The disease name of coronavirus disease 2019(COVID-19) in traditional Chinese medicine(TCM) is "pulmonary epidemic". The disease is caused by the different properties of pathogen such as coldness, dampness, heat, toxin. In the beginning, the core pathogenesis is lungs and spleen disturbed by external pathogen, and there is heat(fire), coldness, dampness, toxin, phlegm, stagnation, knot, stasis, dryness, deficiency and other pathological changes in the pathological process. The method of clearing heat, dispelling coldness, eliminating dampness, removing toxicity, dispelling phlegm, removing stagnation, attacking knot, removing blood stasis, moistening dryness and tonifying deficiency should be given;In the early and middle stages, pathogen is in defensive qi, and the basic treatment principal is to remove heat to clear and disseminate defensive qi. Toxic coldness and tonic products should be used with caution. For the severe or critical type with the pathogen damaging lung collaterals attention should be payed to the application of prescription with cooling blood and dissipating blood, and early intervention of TCM can effectively prevent the disease development.
