妊娠期糖尿病患者行胰岛素注射治疗的孕周对治疗效果及妊娠结局的影响分析

Analysis of the Effect of Gestational Weeks of Insulin Injection Therapy on the Therapeutic Effect and Pregnancy Outcome of Gestational Diabetes Patients

目的 分析妊娠期糖尿病患者行胰岛素注射治疗的孕周对治疗效果及妊娠结局的影响.方法 以2018年1—12月为研究时间段,将该时间段该院收治的妊娠期糖尿病患者86例作为研究对象,按随机数字表法分成两组,各43例,观察组、对照组分别在<30孕周、≥30孕周时接受胰岛素注射治疗,对比其临床效果.结果 分娩前,观察组FPG(5.46±0.52)mmol/L、2 hPG(6.67±0.75)mmol/L、HbAlc(5.57±0.61)%均低于对照组(6.21±0.76)mmol/L、(7.35±0.60)mmol/L(6.48±0.73)%,两组数据差异有统计学意义(P<0.05);观察组发生剖宫产、早产、胎膜早破、羊水过多、产后出血及感染等不良妊娠结局的比率均低于对照组,两组数据差异有统计学意义(χ2=7.283、9.127、10.254、5.124、5.268,P<0.05).结论 妊娠期糖尿病患者在<30孕周时行胰岛素注射治疗,疗效显著,可有效改善妊娠结局.

Objective To analyze the effect of gestational weeks of insulin injection therapy on gestational diabetes patients’ treatment effect and pregnancy outcome. Methods From January to December 2018 as the research period, 86 patients with gestational diabetes admitted to the hospital as the research object during this period were divided into two groups according to the random number table method, 43 cases each, the observation group, the control group, received insulin injection treatment at <30 gestational weeks and ≥30 gestational weeks, and compared their clinical effects. Results Before delivery,FPG(5.46±0.52)mmol/L, 2 hPG(6.67±0.75)mmol/L and HbAlc(5.57±0.61)% in the observation group were lower than those in the control group(6.21±0.76)mmol/L(7.35±0.60)mmol/L(6.48±0.73)%. There was a statistically significant difference(P <0.05);the rate of adverse pregnancy outcomes of cesarean section, premature delivery, premature rupture of membranes,polyhydramnios, postpartum hemorrhage, and infection in the observation group were lower than those in the control group.There was a statistically significant difference in the group data(χ^2=7.283, 9.127, 10.254, 5.124, 5.268, P<0.05). Conclusion Patients with gestational diabetes should be treated with insulin injections when they are <30 weeks of gestation, which has a significant effect and can effectively improve pregnancy outcomes.
