对口切开隧道式切除联合垫棉法治疗复杂性肛瘘的疗效分析

Contraincision&Resection in Tunnel Combined with Cotton-Padding Manipulation in the Treatment of Complex Anal Fistula:Effect Analysis

为探讨对口切开隧道式切除联合垫棉法治疗复杂性肛瘘的临床疗效,将2018年2月至2019年3月我院收治的84例复杂性肛瘘患者随机分为对照组和观察组,各42例,对照组采用传统肛瘘切除术治疗,观察组采用对口切开隧道式切除加垫棉法治疗。比较2组治疗总体效果,术后第2、7、10天疼痛程度(VAS评分),创面愈合时间,以及术后创面愈合时、6个月时患者的肛门功能(Wexner评分)。结果显示,2组治愈率比较差异无统计学意义,P>0.05;观察组术后第2、7、10天时VAS评分均明显低于对照组,P<0.05;观察组创面愈合时间明显短于对照组,术后创面愈合时及术后6个月时Wexner评分均明显低于对照组,P<0.05。结果表明,对口切开隧道式切除联合垫棉法治疗复杂性肛瘘效果显著,患者术后疼痛轻,创面愈合时间快,对肛门功能影响小。

This study was to investigate the clinical effect of contraincision&resection in tunnel combined with cotton-padding manipulation in the treatment of complex anal fistula,radomly divided 84 cases treated in authors’hospital(2018-02-2019-03)into control group(42 cases,received routine resection)and observation group(42 cases,contraincision&resection in tunnel combined with cotton-padding manipulation);then,compared both groups’whole effect,VAS score on pain degree on the 2 nd,7 th and 10 th after surgery,wound-surface healing time,and Wexner score on anal funtion at wound-surface healing,and at 6 months after surgery.As results,in curative rate there was no statistical difference between the two groups(P>0.05);but,in VAS score on the 2 nd,7 th and 10 th day,in wound-surface healing time,and in Wexner score at healing,and at 6 months after surgery,observation group was significantly and respectively lower,shorter and lower than control group(all,P<0.05).Results show that the combined procedure used in observation group for complex anal fistula has significant effect and with lighter postoperative pain and faster wound-surface healing,as well less impact on anal function.
