围术期糖尿病患者血清孤啡肽含量的变化

Serum nociceptin/orphanin FQ concentration in diabetic mellitus patients during perioperative period

目的观察糖尿病患者围术期血清孤啡肽(N/OFQ)含量的变化。方法选取山西医科大学第二医院2019年5~8月在蛛网膜下腔阻滞下行单侧膝关节置换术的患者66例,分为糖尿病组(D组,n=33)与非糖尿病组(N组,n=33),纳入年龄60~69岁,性别不限,ASA分级Ⅰ或Ⅱ级。测定麻醉前30 min(T0)、手术1 h(T1)、手术结束24 h(T2)血糖值、血清N/OFQ的浓度、中性粒细胞计数(NEUT)和中性粒细胞百分比(NEU%)。结果D组T0血糖值、血清N/OFQ浓度、NEUT、NEU%明显高于N组(P<0.05)。D组T0到T1血糖及血清N/OFQ升高幅度比N组大,N组T0到T1血液NEUT及NEU%升高幅度高于D组(P<0.05)。两组T0到T1血清N/OFQ分别与血糖、NEUT、NEU%变化趋势正相关(P<0.05)。两组T0到T2血糖、血清N/OFQ浓度、NEUT、NEU%升高幅度差异无统计学意义(P>0.05)。结论糖尿病患者血清N/OFQ含量升高可能与炎症有关。

Objective To observe the serum concentration of nociceptin/orphanin FQ(N/OFQ)in patients with diabetes during perioperative period.Methods Sixty-six American Society of Anesthesidogists physical statusⅠorⅡpatients,aged 60-69 years,with elective unilateral knee replacement under subarachnoid space in the Second Hospital of Shanxi Medical University from May to August 2019 were selected and divided into diabetic group(group D,n=33)and non-diabetic group(group N,n=33).Blood glucose,serum N/OFQ,neutrophil count(NEUT)and neutrophil percentage(NEU%)were measured at 30 min before anesthesia(T0),1 h after operation(T1)and 24 h after operation(T2).Results The levels of blood glucose,serum N/OFQ concentration,NEUT and NEU%in group D were significantly higher than those in group N at T0(P<0.05).The increase of blood glucose and serum N/OFQ in group D was larger than those in group N from T0 to T1,the increase of blood NEUT and NEU%in group N was higher than those in group D from T0 to T1(P<0.05).The serum N/OFQ of the two groups were positively correlated with the change trend of blood glucose,NEUT and NEU%(P<0.05).However,there was no significant difference in the change of blood glucose,serum N/OFQ,NEUT and NEU%from T0 to T2 between two groups(P>0.05).Conclusion The serum concentration of N/OFQ in diabetic patients were apparently increased,which might be related to inflammation.
