摘要
目的观察青少年2型糖尿病(T2DM)患者血清C1q/肿瘤坏死因子相关蛋白3(C1q/TNF-related protein 3,CTRP3)水平的变化,探讨CTRP3与胰岛素抵抗、糖脂代谢的关系。方法选取新诊断的青少年T2DM患者60例,根据体重指数(body mass index,BMI)分组,BMI≥24 kg/m2为T2DM-超重(OW)/肥胖(OB)组,BMI<24 kg/m2为T2DM-正常体重(NW)组,另选健康的青少年30名为正常对照组。测定血清CTRP3、空腹血糖(FPG)、腰臀比(WHR)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、血脂等指标,稳态模型评估胰岛素抵抗指数(HOMA-IR)。结果青少年T2DM患者血清CTRP3水平低于正常对照组,且T2DM-OW/OB组血清CTRP3水平低于T2DM-NW组(P<0.05);血清CTRP3水平与BMI、WHR、FPG、HbA1c、甘油三酯(TG)、FINS、HOMA-IR呈负相关,与高密度脂蛋白胆固醇(HDL-C)呈正相关;以CTRP3为因变量行多元线性回归分析,结果显示CTRP3水平与BMI、HbA1c、TG、HOMA-IR独立相关。结论青少年T2DM患者血清CTRP3水平降低,其降低程度与胰岛素抵抗、糖脂代谢紊乱、肥胖密切相关。
Objective To investigate serum C1q/TNF-related protein 3(CTRP3)levels in adolescent patients with type 2 diabetes mellitus(T2DM),and explore the relationship of CTRP3 levels with insulin resistance and glycolipid metabolism.Methods 60 adolescents with newly diagnosed T2DM were divided into overweight(OW)/obesity(OB)group(BMI≥24 kg/m2)and normal weight(NW)group(BMI<24 kg/m2)according to body mass index(BMI),and 30 healthy subjects were enrolled as normal control group.Then,serum CTRP3 levels,fasting plasma glucose(FPG),HbA1c,fasting insulin(FINS)and lipid were measured.And the homeostasis model was used to evaluate insulin resistance index(HOMA-IR).Results The serum CTRP3 level in T2DM group were significantly lower than normal control group,and CTRP3 in OW/OB group were also lower compared to NW group.Single factor correlation analysis showed that the level of serum CTRP3 was negatively related with BMI,waist-to-hip ratio(WHR),FPG,HbA1c,triglyceride(TG),FINS and HOMA-IR,while positively related with high density lipoprotein cholesterol(HDL-C).Multiple linear regression showed that BMI,HbA1c,TG and HOMA-IR were independent factor of serum CTRP3.Conclusion The serum CTRP3 level was decreased in adolescents with T2DM,and was closely relevant to IR,glycolipid disorder,and obesity.
