腹股沟疝无张力修补术后早期切口疼痛的相关因素分析

Analysis of related factors of early postoperative pain following tension-free repair of inguinal hernia

目的 :探讨腹股沟疝无张力修补术后发生早期切口疼痛的相关影响因素。方法 :回顾性分析腹股沟疝行无张力修补术患者共434例。依据术后72 h切口疼痛的VAS评分将患者分为疼痛组(VAS>3分)108例及轻疼痛组(VAS≤3分)326例。对两组患者的一般情况、疝分类分型及手术情况进行比较。结果 :疼痛组患者平均年龄、BMI指数明显低于轻疼痛组,平均手术时间明显长于轻疼痛组,使用自黏性补片手术比例明显低于轻疼痛组,两组使用负压引流及术后镇痛的比例有统计学差异(P<0.05)。两组患者性别、合并症、疝分型、疝位置、手术修补材料之间的差异无统计学意义(P>0.05)。结论 :腹股沟疝无张力修补术后早期切口疼痛与患者年龄、BMI指数、手术时间、补片类型、手术引流、术后镇痛具有相关性。

Objective: To investigate the related factors of early postoperative pain following tension-free herniorrhaphy. Methods: The data from 434 patients with inguinal hernia treated by tension-free herniorrhaphy were retrospectively analyzed. The patients were divided into a pain group(n=108, VAS>3) and a light pain group(n=326, VAS≤3) according to the VAS score 72 h after operation. The general condition, hernia classification and operation of the patients were compared between the two groups. Results: The average age, BMI index and the proportion of surgery using self-adhesive patch were lower while the operation time was longer in the pain group than the light pain group. The comparison of the proportion of both the use of drainage tube and the postoperative analgesia showed statistical significance(P<0.05). There were no differences between the two groups in the other factors including sex, complication, hernia type, hernia side and operation mode(P>0.05). Conclusion: Age, BMI index, operation time, patch type, surgical drainage and postoperative analgesia were significantly associated with the occurrence of early postoperative pain.
