摘要
目的 :探讨循环肿瘤DNA(circulating tumor DNA, ctDNA)在胃肠间质瘤诊治中的应用。方法 :选取80例胃肠间质瘤患者,所有患者均接受外周ctDNA检测,通过实时定量PCR(qRT-PCR)和高通量测序法分析胃肠间质瘤基因病变类型与性别、肿瘤原发部位、肿瘤直径、Fletcher分级和核分裂象之间的关系。结果 :ctDNA与性别无明显相关性(P>0.05),ctDNA与肿瘤原发部位、肿瘤直径、Fletcher分级和核分裂象有显著相关性(P<0.05);ctDNA阳性患者无进展生存期低于阴性患者(P<0.05)。结论 :ctDNA可能为胃肠间质瘤患者预后不良因素,可作为动态监测指标,临床意义显著。
Objective:To investigate the application of circulating tumor DNA(ctDNA)in the diagnosis and treatment of gastrointestinal stromal tumors(GIST).Methods:Eighty patients with GIST were selected and underwent peripheral blood ctDNA detection.The relationship between the type of GIST gene lesions and sex,oncology primary site,oncology diameter,Fletcher grade,and mitotic figures were analyzed by real-time quantitative PCR(qRT-PCR)and high-throughput sequencing.Results:There was no significant correlation between ctDNA and gender(P>0.05),but there was significant correlation between ctDNA and oncology primary site,oncology diameter,Fletcher grade and mitotic figures(P<0.05).Progression-free survival was lower in ctDNA-positive patients than in ctDNA-negative patients(P<0.05).Conclusion:CtDNA may be a poor prognostic factor for patients with gastrointestinal stromal tumors and can be used as a dynamic monitoring index and has significant clinical significance.
