半夏泻心汤组方配伍机制研究及治疗慢性胆汁反流性胃炎临床观察

Study on the Compatibility Mechanism of Banxia Xiexin Decoction and Clinical Observation on Its Effect in Treating Chronic Bile Reflux Gastritis

目的探讨半夏泻心汤的组方及药物配伍原理并观察该方加减内服对慢性胆汁反流性胃炎患者的临床疗效。方法选取2018年4月—2019年4月收治的慢性胆汁反流性胃炎患者60例,入组病例均采用信封法随机分为对照组和试验组。对照组30例,应用抑酸、保护胃黏膜、促胃肠动力等常规治疗方法,试验组30例,在对照组的基础上联合半夏泻心汤加减内服治疗方案,观察2组患者治疗前后有效率及中医证候评分变化情况。结果经临床治疗后,试验组有效率为90.0%,中医证候评分(8.1±1.59)分;对照组有效率为66.7%,中医证候评分(11.4±2.3)分;试验组均优于对照组,差异有统计学意义(P <0.05)。结论半夏泻心汤理气和中,疏肝调脾,平调寒热,消痞散结,慢性反流性胃炎患者在常规药物治疗基础上加服半夏泻心汤可有效改善胃动力,促进胃黏膜修复效果,抑制炎症进展及胃酸分泌,减轻胃痛、反酸、嗳气、恶心等不适症状,临床疗效优异,值得临床推广。

Objective To explore the prescription and compatibility principle of Banxia Xiexin decoction and observe the clinical effect of Banxia Xiexin decoction on bile reflux gastritis.Methods 60 patients with chronic bile reflux gastritis were selected from April 2018 to April 2019.The patients were randomly divided into control group and experimental group by envelope method.In the control group,30 cases were treated with acid suppression,gastric mucosa protection and gastrointestinal motility promotion.In the experimental group,30 cases were treated with Banxia Xiexin decoction oral administration on the basis of the control group.Results After clinical treatment,the effective rate of the experimental group was 90%,and the score of TCM symptoms was(8.1±1.59).The effective rate of the control group was 66.7%,and the score of TCM symptoms was(11.4±2.3).The experimental group was superior to the control group,and the difference had statistical significance(P<0.05).Conclusion Banxia Xiexin decoction can effectively improve gastric motility,promote the repair effect of gastric mucosa,inhibit the progress of inflammation and gastric acid secretion,alleviate stomach pain,acid reflux,belching,nausea and so on.The clinical effect is excellent and worthy of clinical promotion.
