中药熏蒸联合肌电生物反馈疗法对膝骨性关节炎患者膝关节功能的影响

Effect of traditional Chinese medicine fumigation combined with electromyographic biofeedback therapy on knee function in patients with knee osteoarthritis

目的观察中药熏蒸联合肌电生物反馈疗法对膝骨性关节炎患者膝关节功能的影响。方法将2018年1月—2019年6月福州市第二医院康复科诊治的75例膝骨性关节炎患者按就诊顺序随机分为熏蒸组、肌电组和联合组,每组25例。熏蒸组在常规康复治疗基础上予中药熏蒸治疗,肌电组在常规康复治疗基础上予肌电生物反馈疗法治疗,联合组在常规康复治疗基础上予中药熏蒸联合肌电生物反馈疗法治疗。治疗4周后,量化评估3组患者膝关节VAS评分、股四头肌和腘绳肌平均肌电值(AEMG)、腘绳肌与股四头肌峰力矩比(H/Q)。结果治疗4周后,3组VAS评分均明显低于治疗前,联合组VAS评分明显低于熏蒸组和肌电组,熏蒸组VAS评分明显低于肌电组,差异均有统计学意义(P均<0.05);3组AEMG均明显高于治疗前,联合组AEMG明显高于熏蒸组和肌电组,肌电组AEMG明显高于熏蒸组,差异均有统计学意义(P均<0.05);3组H/Q均明显低于治疗前,联合组H/Q明显低于熏蒸组和肌电组,肌电组H/Q明显低于熏蒸组,差异均有统计学意义(P均<0.05)。结论中药熏蒸联合肌电生物反馈疗法能显著减轻膝骨性关节炎患者膝痛,改善膝关节屈伸肌力和肌力平衡,提高膝关节功能。

Objective It is to observe the effect of traditional Chinese medicine fumigation combined with electromyographic biofeedback therapy on knee function in patients with knee osteoarthritis(KOA).Methods 75 cases of KOA patients diagnosed and treated in the rehabilitation department of the Second Hospital of Fuzhou from January 2018 to June 2019 were randomly divided into fumigation group,electromyography group and combined group according to the order of admission.The fumigation group was treated with fumigation of traditional Chinese medicine on the basis of conventional rehabilitation treatment,the electromyography group was treated with electromyography biofeedback treatment on the basis of conventional rehabilitation treatment,and the combined group was treated with traditional Chinese medicine fumigation combined with electromyography biofeedback treatment on the basis of conventional rehabilitation treatment.After 4 weeks of treatment,VAS score of knee joint,Average Electromyography(AEMG)of quadriceps femoris and hamstring femoris muscles,and ratio of hamstring to quadriceps femoris peak moment(H/Q)of patients in the three groups of patients were quantitatively evaluated.Results After 4 weeks of treatment,the VAS scores of the three groups were significantly lower than those before treatment,the VAS scores of the combined group were significantly lower than those of the other two groups,and the VAS scores of the fumigation group were significantly lower than those of the electromyography group,the differences were statistically significant(all P<0.05);AEMG in the three groups was significantly higher than that before treatment in the three groups after treatment,the AEMG in the combined group was significantly higher than that in the other two groups,and the AEMG in the electromyography group was significantly higher than that in the fumigation group,the differences were statistically significant(all P<0.05);the H/Q of the three groups was significantly lower than that before treatment,the H/Q of the combined group was significantly lower than that of the fumigation group and the electromyography group,and the H/Q of the electromyography group was significantly lower than that of the fumigation group,the differences were statistically significant(all P<0.05).Conclusion Traditional Chinese medicine fumigation combined with electromyographic biofeedback therapy could significantly improve knee function,relieve knee pain,improve knee flexion and extension muscle strength and balance in patients with KOA.
