社区全科团队签约服务模式对2型糖尿病干预的研究

Study of the interventions of contracted service model for community general practice team on multiple chronic diseases

目的:探讨社区全科团队签约服务模式对2型糖尿病的干预效果。方法:选取2017年2月-2018年9月社区2型糖尿病患者600例,按不同服务模式分为两组,各300例。对照组接受常规干预;研究组接受社区全科团队签约服务模式干预。比较两组干预效果。结果:研究组干预后糖化血红蛋白、餐后2 h血糖及空腹血糖水平均低于对照组,差异有统计学意义(P<0.05)。研究组服务过程评价得分显著高于对照组,差异有统计学意义(P<0.05)。结论:慢性疾病糖尿病患者接受社区全科团队签约服务模式干预,可有效改善血糖水平,利于疾病预后。

Objective:To explore the intervention effect of contracted service model for community general practice team on multiple chronic diseases.Methods:From February 2017 to September 2018,600 cases of patients with type 2 diabetes in the community were enrolled,they were divided into two groups according to different service models,with 300 cases in each group.The control group received conventional intervention.The study group received contracted service model intervention for community general practice team.The intervention effects between two groups was compared.Results:After intervention,the levels of glycosylated hemoglobin,2h of postprandial blood glucose and fasting blood glucose of the study group were all lower than those of the control group,the differences were statistically significant(P<0.05).The evaluation score of service process in the study group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Patients with diabetes mellitus with chronic disease receive intervention of contracted service model for community general practice team,which can effectively improve blood glucose level and benefit disease prognosis.
