建立一种基于高校社区的居家药学服务模式,让药师走入社区、家庭,提供个体化用药指导。从服务对象的筛选、药学数据支撑、药师培训考核、服务内容等方面构建居家药学服务流程。基于高校社区的居家药学服务流程,为药师开展可持续性居家药学服务奠定基础,为药师转型发展提供参考,并适应健康中国2030的发展战略。

We set up a home-based pharmaceutical service model based on the community of colleges and universities,so that pharmacists can enter the community and family,and provide individualized medication guidance.From the aspects of service object selection,pharmaceutical data support,pharmacist training and assessment and service content,we build the process of home-based pharmaceutical service.Based on the process of home-based pharmaceutical care in university community,it lays a foundation for the pharmacists to carry out sustainable home-based pharmaceutical care,provides a reference for the transformation and development of pharmacists,and adapts to the development strategy of healthy China 2030.
