药物治疗管理服务对艾滋病患者用药依从性的影响

Impact of medication therapy management on medication adherence in patients with HIV

目的评估药物治疗管理服务对艾滋病患者用药依从性的影响。方法选取2019年5月至2020年2月首都医科大学附属北京佑安医院81例用药依从性差的艾滋病患者,随机分为观察组(40例)和对照组(41例)。对照组接受常规治疗服务,观察组除常规治疗外,接受药师提供的药物治疗管理(medication therapy management,MTM)服务。观察患者入组干预12周后用药依从性(采用Morisky用药依从性量表评估),患者生活质量(采用EQ-5D-5L生活量表评估)及CD4+T淋巴细胞计数变化情况。结果与基线相比,观察组比对照组的用药依从性改善更大[(5.05±0.90)分比(4.58±0.74)分,P=0.016]。生活质量评分和CD4+T淋巴细胞计数与对照组相比虽有提高,但差异无统计学意义。结论由药师主导的药物治疗管理服务可有效改善艾滋病患者的用药依从性。

Objective To evaluate the impact of medication therapy management on medication adherence in AIDS patients.Methods A randomized controlled study was performed in Beijing Youan Hospital from May 2019 to February 2020.A total of 81 AIDS patients with poor medication compliance were enrolled and randomly assigned to the experimental group and the control group.The control group received conventional treatment services,and the experimental group received medical treatment management services provided by pharmacists.The main observation indicators were medication compliance scores after 12 weeks of enrollment which were evaluated by the Morisky Drug Compliance Scale.And the secondary observation indicators were patients’quality of life which was evaluated by the European Five-Dimensional Life Scale and CD4+T lymphocyte count.Results Compared with baseline,patients in the experimental group showed greater improvement in medication compliance(5.05±0.90 vs.4.58±0.74,P=0.016).Although the quality of life scores and CD4+T lymphocyte counts of the patients in the experimental group were improved compared with the control group,no significant differences were found.Conclusions Pharmacist-led medication therapy management services can effectively improve medication adherence in AIDS patients.
