胃癌晚期患者血脂、脂蛋白代谢及PTX-3、TTF-1、NSE、CYFRA21-1的表达意义

Significance of serum lipid,lipoprotein metabolism and expression of Pentraxin-3,TTF-1,NSE and CYFRA21-1 in patients with advanced gastric cancer

目的探讨胃癌晚期患者血脂、脂蛋白代谢及血清穿透素-3(PTX-3)、甲状腺转录因子1(TTF-1)、神经元特异烯醇化酶(NSE)、人细胞角蛋白21-1片段(CYFRA21-1)的表达意义,为胃癌早、中、晚期的临床诊治提供诊治依据。方法选取胃癌患者137例进行回顾性分析,按病情发展情况将其分成A、B、C组,即早期组、中期组和晚期组,并将三组患者的各项临床指标纳入SPSS21.0软件处理,比较其血脂、脂蛋白代谢及PTX-3、TTF-1、NSE、CYFRA21-1的表达情况。结果经临床检测发现,治疗前C组TG、TC、HDL、LDL、PTX-3、TTF-1、NSE、CYFRA21-1表达异常程度更显著(P<0.05)。C组术后TG低于术前,TC、LDL均高于术前,而PTX-3、TTF-1、NSE及CYFRA21-1表达均低于术前,差异显著(P<0.05)。将以上P<0.05的各项指标纳入多因素Logistic回归分析发现,TG、TC、HDL、LDL、PTX-3、TTF-1、NSE及CYFRA21-1表达情况均与胃癌晚期发病情况有关。与单项诊断及其他联合诊断方式相比,四联(PTX-3+NSE+TTF-1+CYFRA21-1)诊断的敏感度、特异度、准确度均更高(P<0.05)。结论在胃癌晚期患者的临床检测中,TG、TC、HDL、LDL、PTX-3、TTF-1、NSE、CYFRA21-1表达均异常,应根据其表达情况制定科学、合理的干预措施进行干预,提升患者预后生存率。

Objective To explore the significance of serum lipids,lipoprotein metabolism and serum Ptx-3,TTF-1,NSE,CYFRA21-1 expression in patients with advanced gastric cancer,and provide the basis for diagnosis and treatment of early,middle and late gastric cancer.Methods A total of 137 patients with gastric cancer admitted to our hospital from January 2016 to January 2019 were analyzed retrospectively.According to the development of the disease,the patients were divided into group A,B and C,namely 47 cases,45 cases and 45 cases,namely,the early group,the middle group and the late group.The clinical indexes of the three groups were included in spss 21.0 software to compare their blood lipid,lipoprotein metabolism,and the performance of Ptx-3,TTF-1,NSE and CYFRA21-1.Result Before treatment,the abnormal expression of TG,TC,HDL,LDL,Ptx-3,TTF-1,NSE and CYFRA21-1 in group C was more significant(P<0.05).TG,TC,LDL,Ptx-3,TTF-1,NSE and CYFRA21-1 were significantly lower in group C than in group C(P<0.05).The expression of blood lipid(TG,TC),lipoprotein(HDL,LDL),Ptx-3,TTF-1,NSE and CYFRA21-1 were all related to the incidence of advanced gastric cancer.Compared with single diagnosis and other combined diagnosis,PTX-3+NSE+TTF-1+ CYFRA21-1 had higher sensitivity,specificity and accuracy(P<0.05).Conclusion In the clinical detection of advanced gastric cancer patients,the expression of blood lipid(TG,TC),lipoprotein metabolism(HDL,LDL) and Ptx-3,TTF-1,NSE,CYFRA21-1 were abnormal.Scientific and reasonable intervention measures should be made according to their expression to improve the prognosis and survival rate of patients.
