耳揿针对急性胰腺炎腹痛缓解的疗效及作用机制研究

The efficacy and mechanism of ear pressed needle on abdominal pain in acute pancreatitis

目的:探究耳揿针对急性胰腺炎腹痛缓解的疗效及作用机制和安全性。方法:选取我院2017年8月-2019年7月收治的80例急性胰腺炎患者作为研究对象,按随机数字表法分为对照组和研究组各40例。两组均采用常规治疗手段,在此基础上对照组实施常规护理,研究组则在对照组的基础上配合耳揿针进行干预。比较两组治疗前后疼痛数字评分法(Numeric Rating Scale,NRS)各疼痛级别的分布情况,NRS评分和超敏C-反应蛋白(Highsensitivity C-reactive Protein,hs-CRP)、白介素-6(Interleukin-6,IL-6)等炎性因子水平,以及不良反应发生率。结果:护理前,对照组轻度、中度、重度疼痛的占比与治疗组各项相比较,差异无统计学意义(P>0.05);护理后,治疗组轻度疼痛占比高于对照组,中度疼痛占比低于对照组,差异有统计学意义(P<0.05),重度疼痛占比差异无统计学意义(P>0.05)。护理前,两组NRS评分和hs-CRP、IL-6水平相比较,差异无统计学意义(P<0.05);护理后,两组NRS评分和hs-CRP、IL-6水平均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05)。对照组发生不良反应8例,治疗组发生不良反应2例,治疗组低于对照组,差异有统计学意义(P<0.05)。结论:耳揿针应用在急性胰腺炎的临床治疗中,配合相应的护理干预,可有效缓解的腹痛症状,减轻患者的疼痛,并能通过抑制炎性因子的表达,起到良好的治疗效果,安全性有保障,值得推广。

Objective:To investigate the therapeutic effect,mechanism and safety of auricular compression for relief of acute pancreatitis abdominal pain.Methods:80 patients were divided into the control group and research group according to random number table method.Both groups used conventional treatment methods,routine nursing more was applied in the control group,and the research group was given auricular press needle intervention more.Results:The proportion of mild pain in the treatment group was higher than that in the control group,and the proportion of moderate pain was lower than that in the control group,with statistically significant differences(P<0.05),while the proportion of severe pain was not statistically significant difference(P>0.05).Before nursing,NRS scores,hs-CRP and IL-6 levels in two groups were compared,and the difference was not statistically significant(P<0.05).After nursing,NRS scores,hs-CRP and IL-6 levels in the two groups were lower than those before treatment,and those in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).Adverse reactions occurred in 8 cases(20.00%)in the control group.Adverse reactions occurred in 2 patients(5.00%)in the treatment group,which was lower than the control group(P<0.05).Conclusion:The application of auricular press needle in the clinical treatment of acute pancreatitis can effectively relieve the symptoms of abdominal pain,with appropriate nursing interventions,reduce the pain of patients,and by inhibiting the expression of inflammatory factors,play a good therapeutic effect,and the safety is guaranteed,it is worth promoting.
