微创DHS内固定与PFNA内固定治疗老年股骨粗隆间骨折的疗效对比分析

Comparative Analysis of Minimally Invasive DHS Internal Fixation and PFNA Internal Fixation in the Treatment of Elderly Femoral Intertrochanteric Fractures

目的探究治疗老年股骨粗隆间骨折采取微创DHS内固定与PFNA内固定的疗效。方法依据患者入院就诊时间将该院自2018年9月—2019年10月期间接受手术的88例老年股骨粗隆间骨折患者随机分为试验组(n=44)与参照组(n=44),试验组患者实施PFNA内固定治疗,参照组患者实施DHS内固定治疗,对比两组患者手术情况、手术效果及并发症发生情况。结果试验组患者手术出血量为(214.28±15.36)mL比参照组的(306.25±20.63)mL少(t=23.719,P=0.000),手术时间为(64.13±10.26)min比参照组的(77.32±11.62)min短(t=5.644,P=0.000),术后首次活动时间为(17.27±2.14)min比参照组(21.65±2.47)min短(t=8.890,P=0.000),住院时间为(14.26±3.95)d比参照组(19.15±4.68)d短(t=5.296,P=0.000),并发症发生率(4.54%)明显低于参照组(22.73%)(χ~2=6.175,P=0.013),治疗有效率(97.73%)明显高于参照组(81.82%)(χ~2=6.065,P=0.014),组间差异有统计学意义(P<0.05)。结论 PFNA内固定治疗老年股骨粗隆间骨折疗效显著,手术出血量少,能有效缩短手术时间及住院时间,安全性更高。

Objective To explore the effect of minimally invasive DHS internal fixation and PFNA internal fixation for the treatment of elderly femoral intertrochanteric fractures.Methods According to the time of admission,88 elderly patients with intertrochanteric fractures who underwent surgery in the hospital from September 2018 to October 2019 were randomly divided into a test group(n=44)and a reference group(n=44),Patients in the test group were treated with PFNA internal fixation,and patients in the reference group were treated with DHS internal fixation.The surgical conditions,surgical effects,and complications of the two groups were compared.Results The amount of surgical bleeding in the test group was(214.28±15.36)mL less than that in the reference group(306.25±20.63)mL(t=23.719,P=0.000),and the operation time was(64.13±10.26)min shorter than the reference group's(77.32±11.62)min(t=5.644,P=0.000),the first time of postoperative activity was(17.27±2.14)min shorter than the reference group(21.65±2.47)min(t=8.890,P=0.000),and the hospital stay was(14.26±3.95)d shorter than the reference group(19.15±4.68)d(t=5.296,P=0.000),the incidence of complications(4.54%)was significantly lower than that of the reference group(22.73%)(χ2=6.175,P=0.013),and the effective rate of treatment(97.73%)was significantly higher than that of the reference group(81.82%)(χ2=6.065,P=0.014),and the difference was statistically significant between two groups(P<0.05).Conclusion PFNA internal fixation is effective in the treatment of elderly femoral intertrochanteric fractures,and the amount of surgical bleeding is small.It can effectively shorten the operation time and hospitalization time,and has higher safety.
