经皮椎间孔镜治疗腰椎间盘突出症的临床疗效分析

Analysis of Clinical Efficacy of Percutaneous Endoscopic for the Treatment of Lumbar Disc Herniation

目的观察并探讨腰椎间盘突出症(LDH)应用经皮椎间孔镜(PTED)技术治疗的临床应用效果,通过分析经皮椎间孔镜(PTED)技术的治疗优势,总结腰椎间盘突出症(LDH)的临床经验。方法选取该院2016年7月—2019年6月收治的50例应用PTED技术治疗的单一节段腰椎间盘突出症(LDH)患者作为观察对象,进行回顾性分析。观察患者治疗前后腰部与下肢的VAS疼痛评分情况,比较患者治疗前后的Oswestry功能障碍(ODI)分值,对患者的末次随访进行记录,分析临床治疗效果,计算优良率。结果术后24 h、3 d、1个月、3个月、6个月,50例患者的腰部、下肢VAS评分与ODI分值,明显低于术前,组间差异有统计学意义(F=1 316.245、1 869.452、2041.674,P<0.05)。50例患者术后6个月的优良率为94%,明显优于术后1个月、3个月的86%与90%,组间差异有统计学意义(χ~2=5.741,P<0.05)。结论经皮椎间孔镜(PTED)技术治疗腰椎间盘突出症(LDH)效果较好,患者术后伤口小、术中出血少、术后恢复快,安全性较高。

Objective To observe and discuss the clinical application of lumbar disc herniation(LDH)using percutaneous intervertebral foraminal(PTED)technology,and analyze the advantages of percutaneous intervertebral foraminal(PTED)technology to summarize the lumbar disc clinical experience with Prolapse(LDH).Methods Fifty patients with single-segment lumbar disc herniation(LDH)treated with PTED technology from July 2016 to June 2019 in the hospital were selected as observation objects for retrospective analysis.Observe the VAS pain scores of the waist and lower limbs before and after treatment,compare the Oswestry Dysfunction(ODI)scores of the patients before and after treatment,record the patient's last follow-up,analyze the clinical treatment effect,and calculate the excellent rate.Results At 24 h,3 d,1,3,and 6 months after surgery,the waist and lower limb VAS scores and ODI scores of 50 patients were significantly lower than those before surgery,and the differences between the groups were statistically significant(F=1316.245、1869.452、2041.674,P<0.05).The excellent and good rate of 6 months after operation in 50 patients was 94%,which was significantly better than 86%and 90%at 1 and 3 months after operation.The difference between the groups was statistically significant(χ2=5.741,P<0.05).Conclusion Percutaneous transforaminal(PTED)technique is effective in treating lumbar disc herniation(LDH).The patient has a small postoperative wound,less intraoperative bleeding,rapid postoperative recovery,and higher safety.
