改良髂筋膜间隙联合腘窝坐骨神经阻滞在单侧大隐静脉曲张手术中的麻醉效果

Anesthesia Effect of Modified Iliac Fascial Space Combined with Popliteal Sciatic Nerve Block in Unilateral Great Saphenous Varicose Vein Surgery

目的分析单侧大隐静脉曲张术中腘窝坐骨神经阻滞联合改良髂筋膜间隙的应用效果。方法选取2017年6月—2019年6月在该院行单侧大隐静脉高位结扎加抽剥术的70例患者为研究对象,随机分为两组,对照组行腰硬联合阻滞,观察组行改良髂筋膜间隙联合腘窝坐骨神经阻滞,对比两组麻醉效果以及不良反应发生情况。结果观察组麻醉优良率为94.29%,接近于对照组的97.14%(χ^2=0.348,P>0.05);观察组麻醉术中循环稳定率为97.14%,高于对照组的65.71%(χ^2=11.431,P<0.05);观察组不良反应发生率为0.00%,低于对照组的22.86%(χ^2=9.032,P<0.05);与对照组对比,观察组的术后VAS评分更低(t=27.528,P<0.05)。结论单侧大隐静脉曲张术中腘窝坐骨神经阻滞联合改良髂筋膜间隙的应用效果理想。

Objective To analyze the effect of popliteal sciatic nerve block combined with improved iliac fascial space in unilateral great saphenous vein varicose veins.Methods A total of 70 patients who underwent unilateral great saphenous vein ligation plus stripping in the hospital from June 2017 to June 2019 were selected as the research subjects.They were randomly divided into two groups,and the control group underwent combined lumbar and hard block.The modified iliac fascial space combined with popliteal sciatic nerve block was performed in the group.The anesthesia effect and adverse reactions were compared between the two groups.Results The excellent and good rate of anesthesia in the observation group was 94.29%,which was close to 97.14%in the control group(χ^2=0.348,P>0.05);the circulation stability rate in the observation group during anesthesia was 97.14%,which was higher than 65.71%in the control group(χ^2=11.431,P<0.05);the incidence of adverse reactions in the observation group was 0.00%,which was lower than22.86%of the control group(χ^2=9.032,P<0.05);compared with the control group,the postoperative VAS score of the observation group was lower(t=27.528,P<0.05).Conclusion The effect of popliteal sciatic nerve block combined with improvement of iliac fascial space in unilateral great saphenous vein varicose veins is ideal.
