颈椎前路减压固定治疗急性颈椎间盘突出致脊髓损伤的效果研究

摘要 目的探究颈椎前路减压固定治疗急性颈椎间盘突出致脊髓损伤的临床治疗效果。方法选取2016年7月—2019年6月间该院收治的42例急性颈椎间盘突出致脊髓损伤患者作为该次实验对象,按照治疗方法的不同平均分为两组,即实施后路减压侧块钢板内固定植骨术治疗的对照组和实施颈椎前路减压固定治疗的观察组,观察两组患者脊髓神经功能和颈椎功能改善状况,并评估临床治疗有效率,对比临床疗效。结果两组患者术前颈椎JOA评分差异无统计学意义(P>0.05),术后观察组JOA评分(14.33±1.12)分明显优于对照组的(9.71±0.89)分(t=14.800,P<0.05);两组患者术后脊髓功能A级和B^D级差异无统计学意义(P>0.05),但观察组E级占比(90.48%)明显高于对照组(57.14%)(χ^2=6.035,P<0.05);观察组治疗总有效率(95.24%)更优(χ^2=6.929,P<0.05)。结论给予急性颈椎间盘突出致脊髓损伤患者颈椎前路减压固定治疗能促进患者颈椎及脊髓神经功能的恢复。 Objective To explore the clinical treatment effect of anterior cervical decompression and fixation on spinal cord injury caused by acute cervical disc herniation.Methods Forty-two patients with spinal cord injury caused by acute cervical intervertebral disc herniation treated in our hospital from July 2016 to June 2019 were selected as the subjects of this experiment.They were divided into two groups according to different treatment methods,Namely,the control group who underwent posterior decompression lateral mass plate internal fixation and bone grafting and the observation group who underwent anterior cervical decompression and fixation therapy observed the improvement of spinal nerve function and cervical spine function in both groups,and evaluated the effectiveness of clini-cal treatment and compared clinical efficacy.Results The JOA score of cervical spine before operation in both groups was not statisti-cally significant difference(P>0.05).The JOA score of the observation group after operation(14.33±1.12)points was significantly better than the control group(9.71±0.89)points(t=14.800,P<0.05);A and B-D spinal cord function of the two groups of patients were not statistically significant difference(P>0.05),but the proportion of E in the observation group(90.48%)was significantly higher than that in the control group(57.14%)(χ^2=6.035,P<0.05);the total effective rate of treatment in the observation group(95.24%)was better(χ^2=6.929,P<0.05).Conclusion Anterior cervical decompression and fixation for patients with spinal cord injury caused by acute cervical intervertebral disc herniation can promote the recovery of the cervical spine and spinal nerve function.
