摘要
目的对呼吸窘迫综合征(RDS)早产儿实施加温湿化高流量鼻导管通气(HHFNC),分析临床价值。方法于2017年3月—2018年4月期间抽取94例RDS早产儿,以数字随机表法分成研究组及参照组,各47例,参照组行鼻塞式持续性气道正压通气(NCPAP),研究组行HHFNC,对两组干预结果进行观察。结果治疗前,两组PaO2、SaO2、PaCO2水平相比,差异无统计学意义(P>0.05),治疗后3 d,研究组PaO2、PaCO2与参照组相比,差异有统计学意义(P<0.05),治疗后3 d,两组SaO2水平差异无统计学意义(P>0.05);研究组无创通气时间为(6.58±1.34)d,与参照组的(6.24±1.26)d相比,差异无统计学意义(t=1.267,P>0.05);两组院内感染发生率、气胸发生率相比,差异无统计学意义(P>0.05),研究组鼻部损伤、CO2潴留、腹胀发生率与参照组相比,相对更低,差异有统计学意义(P<0.05)。结论HHFNC可有效改善RDS早产儿血气指标,还可降低鼻损伤、腹胀等并发症风险,值得应用。
Objective To implement warming and humidifying high-flow nasal catheter ventilation(HHFNC)in preterm infants with respiratory distress syndrome(RDS)to analyze the clinical value.Methods From March 2017 to April 2018,94 cases of RDS premature babies were selected and divided into a study group and a reference group of 47 cases by digital random table method.In the study group,HHFNC was conducted to observe the intervention results of the two groups.Results Before treatment,the levels of PaO2,SaO2,and PaCO2 in the two groups were not statistically significantly different(P>0.05).Three days after the treatment,PaO2 and PaCO2 in the study group were statistically significantly different from the reference group(P<0.05).Three days after treatment,the difference in SaO2 levels between the two groups was not statistically significant(P>0.05);the noninvasive ventilation time of the study group was(6.58±1.34)d,which was not statistically significant compared to(6.24±1.26)d of the reference group(t=1.267,P>0.05).There was no statistically significant difference in the incidence of nosocomial infection and pneumothorax between the two groups(P>0.05).The incidence of nasal injury,CO2 retention,and bloating in the study group were respectively compared with the reference group,relatively lower,the difference was statistically significant(P<0.05).Conclusion HHFNC can effectively improve the blood gas index of preterm infants with RDS,and can also reduce the risk of complications such as nasal injury and abdominal distension.It is worth applying.
